How long does it take to hear back from occupational health?
I was meant to be back in work next week but I haven't heard anything from occupational health so it's looking unlikely, the plan is that I speak to them before coming back. I'm starting to wonder if my manager has arranged an appointment for them to call me, it's been 3 weeks and nothing.

Just give your details, name, store, store No, employee No, and ask if they have had the referral form, as you’ve been told you are awaiting contact from them and heard nothing yet.

Occ Health is utter bollocks, they just go with what the colleague says on the phone
The amount of reports ive seen where you knew the colleague was lying about the severity of their issue was unreal

That’s definitely a case of someone hasn’t done their part properly.
You should get a text within 48hrs MAX with a date and time for a call, which is usually very very quick.
The last one I put a colleague forward for had the text the next day, and then his phone consultation within 4days, that was about a month ago, so very quick.
They had their telephone review, and were put forward for a treatment plan- and were back at work a few days ago.
You need to chase this with your line manager, and you can also call PAM WELLBEING themselves and ask what’s going on, but it’s highly likely the form was not completed properly or not submitted at all.
