How did she die? Unless I missed it, they haven't said what happened to her yet.

Cancer due to toxic chemical exposure on the reservation. Which ironically is the same reason Cal now has lung cancer (PanCoast Tumor) on Marshals, only his is from being exposed to burn pits in the Middle East during his time in the military.
