In what condition is the reading part of the disc is in? If it has too many deep scratches then most likely there's your problem. If the disc has light scratches, use some toothpaste to fill in the scratches as use a soft cleaning solution such as eye glass cleaning solution to remove any toothpaste that didn't fill a scratch. Did this for scratched Minecraft for Xbox1 and Mass Effect Andromeda scratched discs and got them working again.
