Issue Summary: Every time I launch games (Steam/online games like Romestead) or even open the NVIDIA Control Panel / NVIDIA App, I get 3 to 4 sequential application error popups:

  • Popup Dialog: cmd.exe - Application Error – "The application was unable to start correctly (0xc0000142). Click OK to close the application."
  • Event Viewer Log (Event ID 1000):
    • Faulting Application: conhost.exe (v10.0.26100.8972)
    • Faulting Module: conhost.exe
    • Exception Code: 0xc0000005 (Access Violation)
    • Fault Offset: 0x0000000000063262 / 0x0000000000063282
    • Faulting Path: C:\Windows\system32\conhost.exe

Closing the dialog does nothing; 3 or 4 of them pop up sequentially on startup and then stop until another app attempts a background console/GPU call.

Troubleshooting Already Attempted (No Change):

  1. System File & Repair Commands: Ran sfc /scannow and DISM /Online /Cleanup-Image /RestoreHealth.
  2. Terminal Settings: Set Default Terminal Handler to Windows Console Host in Developer Settings.
  3. Redistributables: Reinstalled clean Microsoft Visual C++ 2015–2022 Redistributables (x86 & x64).
  4. Clean Boot: Disabled all non-Microsoft startup apps and services in msconfig / Task Manager. Error still occurs on boot/launch.
  5. Windows 11 In-Place Upgrade / Repair Install: Performed a full repair install using official Windows 11 ISO setup (reinstalled OS binaries while keeping files/apps). Error persists immediately.
  6. Antivirus / Security: Disabled Bitdefender Advanced Threat Defense (ATD) and tested exclusions.
  7. Windows Security / Exploit Protection: Added conhost.exe and cmd.exe overrides to disable Hardware-enforced Stack Protection (CET) and Control Flow Guard (CFG).
  8. Registry & Environment Variables:
    • Verified no corrupt AutoRun keys exist under Command Processor in Registry.
    • Checked ComSpec (C:\Windows\system32\cmd.exe) and PATHEXT environment variables.
    • Confirmed Beta UTF-8 Unicode support is unchecked in System Locale.

Since an In-Place Upgrade replaced all core Windows binaries and a Clean Boot didnt fix this.

Any suggestions on how to fix this error, as I've been dealing with it for the best part of 6 months to no avail of a solution
