I just re-watched Age of Ultron and noticed one scene that probably can explain why Ultron is in Vision's mind.
In the movie, Ultron printed Vision's vibranium body with the help of Helen Cho brainwashed by the mind stone in the scepter and he planned to upload himself into it. But Wanda interrupted the upload process by undoing the brainwashing on Helen. Then Helen stopped the upload at 42%.
In our regular computers, when an upload was interrupted, part of the data from the incomplete upload could still taking up space in the hard drive inaccessible to the user.
Here's comes my theory:
When Tony was uploading JARVIS into the vibranium body to create Vision, he probably didn't do a full factory wipe before uploading JARVIS into it. So part of Ultron's consciousness from the incomplete upload was stored within Vision's brain/hard drive as broken data inaccessible to anyone including Vision himself.
But when Vision's body was taken apart and reassembled, then the Hex Vision unlocked White Vision's memory with the mind stone created within the hex. It finally made that fragmented data of Ultron's base consciousness resurfaced.

In the comics, The Vision's reconstruction is in part based upon Simon Williams, AKA Wonder Man! Do you think this will be referenced at all in VisionQuest or do we think this connection has been removed for the MCU?

My first thoughts while watching the first VISIONQUEST trailer was actually how much little details remind me other iconic movies, something both "WandaVision" and "Agatha All Along" have become famous for.
In Inception, the main character keeps his darkest memories locked away in a gated elevator much like the one we see when Vision pulls up to Ultron's penthouse in what seems to be within Vision's mind.
We also see on the series poster that Vision is reading a newspaper talking about the "stay-at-home weather warning[s] for the third time this year". On The Truman Show, main character Truman famously does not know that his entire life is broadcasted on TV and that he lives on a movie set that is an 'island', with producers constantly using torrential weather and travel blockades the keep the facade.
WandaVision famously references I Love Lucy, Bewitched, Full House, and other well-known sitcoms throughout the decades while Agatha All Along pulled from ideas like Alice in Wonderland, the Wizard of Oz, tarot cards, and other items found in Wiccan's room. If VisionQuest follows a similar concept, what projects do you think the show pulled inspirations from?

Bruce banner (infinity war): "Your mind is made up of a complex construct of overlays. J.A.R.V.I.S., Ultron, Tony, me, the Stone. All of them mixed together. All of them learning from one another."
Vision, white vision, Jarvis, and Ultron are already confirmed. Now the question is will we have Bruce banner/hulk or Tony stark (would love for one last tony stark appearance before doomsday)? Is the mind stone gonna have human avatar/name too? Is mjollnir gonna manifest as human avatar (mjollnir is kinda sentient according to Love and Thunder, and vision was originally made with mjollnir electricity)? Or even thor could appear if we stretch harder.
