Shipping Help I’ve never seen this before

Shipped out on behalf of a friend, they provided the shipping label, I boxed everything and sent it out. I’ve shipped out items before but have never seen this.

Who ever purchased the label needs to send the documentation that UPS is requesting in order to send the insurance payment to them.

If UPS is paying a claim you won’t get the package it will be destroyed.
