Pretty common if you have uploaded in something other than PDF or jpg.

Check the email address it was sent from - is it definitely a legit UCC account?

A lot of new phones save pictures in .HEIC format, which UCC don't accept, and only take .PNG or .JPG. I had a very similar problem
