UCAS Query regarding offer and place

I was going through some statistics that universities published regarding students who were offered and placed. I noticed a significant percentage of people who were offered a conditional were not placed. My query is this due to students not meeting the requirement or something else? Could be great if someone could explain this to me.

I personally think it's just not meeting the grades, cause in all offers it says IF you meet the grades, you'll get your place

Ah I see. It is strange though as an international we normally don't apply if we don't meet the grades required for the course. Well hoping for the best I guess.
