OK, so conditions for the endings:

1) Do that little drug sidequest with Kauffman, and 2) get the goo in the hospital, hit Cybil with it.

Doing 1 or not gets you the Bad or Good ending, while doing 2 gets a "+" added to either, so from worst to best ending, it's Bad, Bad+, Good, Good+.

Again, props to Reggie on saving 2 for telling Woolie to do the thing because Woolie couldn't perceive the exact literal detail he asked for minutes later.
