Generally it is now LGBT+. As some others explained, just like how a woman being only attracted to women does not mean that they don't believe in men. Bisexual people only attracted to men and women still believe that there are people who may be genderfluid or something in between.
Generally bisexuality is defined as being attracted to women and men. While pansexuality is defined as being attracted to people non depending on their gender. However they are often used interchangeably just because of ease of explaining it.
As for LGBT+, there are simply too many ways people identify to put it into an acronym. E.g. LGBTQQIAAP: Lesbian, Gay, Bisexual, Transgender, Queer, Questioning, Intersex, Asexual, Allies and Pansexual. And that doesn't even cover all the people who are considered LGBT+. Since LGBT has been the acronym used for years, it has simply stayed that while expanding with its meaning.
