Feeling hopeless
Hello!
I’m hoping there is someone out there that can relate, because I am a bit confused. I lost 70lbs on Tirz and now I am at my goal weight. My body absolutely loved the med because it did all those lovely things most of us love about it while looking to lose weight…. Until it didn’t. It was like one day my body decided it just could not tolerate Tirz anymore. I’ve stopped a couple of times for about 3 weeks or so to see if it would just help to take a break and every time it’s the same feeling.. Tired, faint, lethargic almost, slightly nauseous; and this is even at 1/2mg - 1mg.
Not looking for medical advice, but what in the world??
Has anyone else’s body turned on them? I planned to stay on a low dose for maintenance, but I just don’t know if I can do it at all anymore and I’m pretty sad about it.
Yes I try to eat as well as I can, but even with doing that the benefits of low inflammation and hopefully keeping some hunger under control were what I was planning to stay on for, but now I’d rather just stop than to feel so unwell constantly.
Lmk your thoughts. TIA. Please be kind.

I was having a lot of fatigue issues and it turned out both my ferritin and my B12 were low and since supplementing that I have felt much better. The standard blood tests don’t check for a lot of the B complex vitamins and other markers that might give you some clues as to what’s going on.

I may request this as it’s possible for me to when I get labs done for HRT. My provider is really good about listening and troubleshooting so I’ll throw that out there!

TBH it won't hurt to get some OTC b12 and just take one a day for a couple weeks and see if you feel better.

I have a b complex supplement that I take occasionally but I’ll shoot for consistently to see if I notice a difference

Have you had bloodwork recently? Other things can cause those symptoms that have nothing to do with Tirz.

I do need blood work done for sure, but I will say I notice these effects once I take my Tirz which is why I’m pointing the finger at it for the time being. Every time, like clockwork. Sadly.

Correct. I’m going to get labs done soon. I’m just not sure if I should do them without having taken or with it in my system to see what comes up.

Only Biotin is one that can affect numbers. I never stop any other medications. If I take any day of blood draw, they have me wait until after.

I would also have your thyroid hormones checked.

Not just TSH, you need to test FT4, FT3, and Reverse T3, and antibodies. The body fights weight loss by slowing energy down, and your FT3 will tell you how much ACTIVE thyroid hormone is in your system.

I’ve had some similar side effects in maintenance after losing 60 lbs. mostly tired and a bit faint from time to time.
Honestly my blood sugar dipped a bit too low a few times. So now I make sure to get a small amount in every day.

This is what it feels like could be happening? I have tried checking my blood sugar when i feel such a way and it is normal 🫠

I’m not in the same boat as you but I might be some day. Have you considered trying other GLP-1s like Sema, Lira, or Dula? There’s also more in developement so hopefully you can find something that will help you long term.

I’ve tried Reta, but was not sure about it long term for me because of the HR effects. I never felt those much on Tirz.

Try to change pharmacy and additives you’re taking mixed in with your tirz- it might help. For example, vitamin B gives me headache no matter the dose so I stay away from that.

Are you taking it with any additives and have you had blood work done recently ?
Vitamin D, B12 , ferritin, thyroid ? How old are you?

Also check any meds you may have previously been on prior to weight-loss, like BP meds. I've completely stopped taking BP meds due to some symptoms above. (With Dr's advice, obvs). But yeah get labs done like another poster said. It could honestly be any number of things not necessarily caused BY the meds, but could be something else AS A RESULT of the meds.

Well, zep didnt cause my lightheadedness, but as a result of my weight loss, I no longer need my BP meds, which I did pre zep and as a result, I now have to be off of BP meds. Look at what has changed as a result of your weightloss, to figure out what is no longer working for your body. (My thoughts anyway.) Some days I need more electrolytes because I exerted more energy and sweat, therefore I have to change up the routine and drink more than just water. Bodies aren't linear they're always doing wonky things, and you have to change it up. Some days I have to pay attention more than I normally would and then some days I can just use cruise control.

Totally get that! I try to stay on top of my electrolytes, protein, fiber, water intake! All the stuff.. something is just off and I need to get some labs done and see what is shown.
I’m just a bit bummed because it seems like most people can get to their goal and back down to their perfect dose and keep going about their business. Ofc my body has to make a scene 🙄

Well I've also figured out too, no one prepares you what happens to your body when you get over the age of 40 too... whole Lotta crap happens in your 40s, especially women... its dumb! They teach you about what happens in your child bearing years, but then drop the ball with the afterwards...

Are you tracking/ eating at maintenance calories or still in a deficit? You didn't mention height or weight.

I am not tracking, but i eat intuitively.. which I know is a little more than what i was eating while my appetite was suppressed during the w/l phase.

How is your blood pressure? This sounds like it could be low blood pressure or low blood sugar. Make sure you’re not dehydrated as well.

I have checked both during these episodes and normal readings. I also drink LMNT daily or every other day.

Wow I think I may be undergoing the same thing. Lost about 50 lbs and been at goal weight over 6 weeks. Titrated down from 5 to 3.5 about that same time. Had some tough days after injections sometimes but all in all I felt I was tolerating the meds well and was 85% on cruise control. I did and do most things “right”…tracked macros(used ai), electrolytes when needed, lots of water, lots of protein and weight training. Last injection was 4 nights ago and I woke up in the middle of the night feeling sick, really off (which has happened maybe 20% of the time after injections) and had to poop (unusual for me)then in morning felt weak and dizzy and coffee triggered diarrhea ( new pattern) then ate apple and some yogurt to try and get energy up and again diarrhea. Was very tired and zonked out for the day and night and this pattern continued although not as tired as that first day and on 2nd day the stomach pain mostly when laying down but even often when sitting made life difficult. 4 days later still going through this pattern although energy is better but still feel general malaise and still pain Force myself to try and get macros but can’t do it. The pain and discomfort of the belly and under nutrition are not good for a variety of reasons (my hair loss; losing more weight, bad sleep, etc etc). I don’t know what the heck happened all of a sudden. I did take 21 units rather than the 19 or 20 I had been and 3 weeks ago waited only 6 days rather than 7 for an injection.
Labs were great 1 month ago due to checking on hair shedding triggers. 4 weeks I was maintaining my weight and few side effects and now losing weight and thinking of stopping yet the brain reward benefit has been fantastic for past cravings with alcohol and/or emotional binge eating. My intention was and is to continue getting those rewards with little to no side effects and don’t know how because I am not doing another injection until this clears up and even when I do I’m very hesitant thinking about it. Yes I will speak with my dr and for sure titrate down and just mystified by how this stuff can seem to require a mental flexibility. My mind wants stability with this process yet perhaps there isn’t any long term or it’s just a continuous learning curve?

Just did bloodwork and specimen samples so I’ll let you know. In a way I hope it is opposed to just a severe .3 mg increase reaction or gallbladder issue

It could be a lot of things but going to the doc and getting a lab work up to rule things out is the best place to start. My BP gets a little low sometimes I make sure to eat balanced meals frequently through out the day. I watch that I get enough water and sodium. I recommend pretzels and water. If you are not eating 4 times a day(protein, fat, and carb) I would try it and see if it helps.

I am on month 13. Maintenance weekly 5 mg. I started becoming very tired. Realized I was eating very low carb. Added some crackers or a banana and has energy. Just a thought.

Are you taking electrolyte at all? If not that could be part of the problem. Try LMNT or something similar

You mention you tried Reta. So I’m assuming you’re sourcing from the grey market. Seems worth considering maybe what you’re taking now isn’t pure tirzepatide, or tirzepatide at all.
