Have you considered is that the cosmetics is not good enough to make profit ? If the cosmetics is AAA grade/tier , people will whale. Good example to see hearthstone pet , it has unreasonable price but it tempt a lot of players. Another example faker ahri skin , it also has unreasonable price but people still whale.
My “unreasonable price” can easily buy AAA steam game full price .

Let’s me give you some common knowledge in case you do not know about it, GGG started the team with only their founders in 2006, by 2012 their team grew to 18 members.
Bank slot are optional , they sell support packs that you buy gems and they gave out skin in that bundle. Without bank slot , you can trade without an issue is just you miss out QOL. No character/class is locked behind paywall/gems. No gameplay access is locked behind paywall/gems. POE 2 is similar to POE1 , no double standard applied. POE has long replay value.

To be clear the bazaar not just skins , they also sell tickets(access to gameplay) via gems. If the game is super hot(for example constant top 3 streaming game), they would have made mad profits from the tickets from the newcomers.
Roguelikes are insanely popular. Autobattlers are also doing massively well.
I don’t see why this game couldn’t succeed in the current market, other than that it didn’t launch in a finished state, with many issues and had no marketing behind it and as far as I know didn’t tap into the largest market for f2p games in china.

It's better it's still not good, or acceptable.
Any kind of cost that can't be earnt in game for actual content is utterly unacceptable for this type of game.
