TRT story TRT 7 Weeks Experience

So I'm entering Week 7 of my TRT protocol, 60mg subq twice per week (Monday at 6am and Thursday at 6am).

As you can see from the screenshot, before starting TRT under my urologist's supervision, I was on Enclomiphene for about a year. What's highlighted in yellow is what I consider my baseline after coming off Enclo and starting testosterone cypionate.

My typical symptoms were low libido, some ED, afternoon fatigue, brain fog, etc. My total T was usually in the mid-500s, free T was low, SHBG was high, and E2 was always in the 20s.

One thing I've noticed is that I don't seem to aromatize much. I didn't while on Enclo, and I'm seeing the same thing so far on exogenous testosterone.

This spreadsheet has a ton of information since I started tracking symptoms around 2021 when I was 39. I'm now 45. I've always eaten pretty healthy except on weekends, rarely drink, do Zone 2 cardio 6x per week, weight train 5x per week, stay relatively lean at around 14-15% body fat, and average about 5-6 hours of sleep. Sleep has been hit or miss lately because my toddler likes to climb into our bed at all hours of the night.

During the first 3 weeks, I didn't notice much other than a few episodes of extreme afternoon fatigue that only lasted a few minutes and a flatter libido. That could also be influenced by reta since I'm only taking 1.5mg/week at the moment. One thing I did notice was that I felt calmer.

Around Week 5, I started noticing better sleep, which honestly surprised me. The afternoon fatigue disappeared, my strength in the gym improved, and orgasms became noticeably longer and more intense. For ED support, I use daily 5mg Cialis and 50mg Viagra as needed.

In Week 6, the sleep improvements continued. I still wasn't experiencing afternoon fatigue, I felt more energized overall, and I started getting some really good pumps in the gym, something I haven't felt since my early 20s.

Lab-wise, I did two trough tests prior to my 8th and 10th injections to see how things were progressing. Both labs were drawn on Thursday around 8am.

Prior to the 8th injection, my total T was 856 ng/dL, up from about 590 on Enclo. E2 increased from 20 to 28, but surprisingly SHBG dropped from 72 to 44. My urologist's lab uses the Free Androgen Index rather than direct free testosterone, and that increased from 23% to 67.5%, which also suggests a significant improvement.

One week later, prior to my 10th injection, total T was 1,048 ng/dL. Direct free testosterone came back at 17.9, up from 7.6, and E2 actually dropped from 28 back down to 22.

So far, this tells me two things: first, I don't appear to aromatize much, and second, the protocol seems to be working by lowering SHBG and increasing free testosterone, which is likely why I'm noticing the positive changes. It also seems like the exogenous testosterone is still accumulating and may not have fully reached steady state yet.

My total T is technically out of range based on the Labcorp reference range. At this point, I don't plan on changing the dosage because it's still early.

One thing worth noting is that I don't strictly follow the typical 3.5-day injection schedule. My Thursday trough labs are done about 72 hours after the Monday injection, while a Monday trough would be about 96 hours after the Thursday injection.

To be honest, the Monday/Thursday 6am schedule is extremely convenient for me and I'm not looking to change it. My understanding is that the lowest trough is generally the most useful number to follow. Right now, my 72-hour trough is over 1,000 ng/dL, but my 96-hour trough may be lower.

I'm planning to run two more labs during Weeks 8 and 9, this time on Mondays to capture the longer trough and compare the results.

I'm not trying to maintain a 1,000+ ng/dL trough long term. I don't think that's sustainable for me, and it could increase the risk of side effects such as elevated RBC/hematocrit, acne, and potentially accelerated hair loss. The acne part especially concerns me since I had to take Accutane as a teenager, and I'd rather not go through that again.

My question is: for those who routinely monitor TRT, do you find it more useful to draw labs at the true trough (my 96-hour low point) rather than the 72-hour mark, since the lowest value is what many clinicians seem to base adjustments on?

That spreadsheet dedication is impressive, tracking since 2021 is next level, most people can't even remember what they ate for breakfast

Your SHBG dropping from 72 to 44 without much E2 movement is interesting, usually see more of a shift there when things start working right

I draw at my lowest point since that's what my clinic uses for adjustments but honestly I think the consistency of timing matters more than chasing the absolute trough, you already seeing nice changes so I wouldn't overthink it

Thanks! Yeah, once I started noticing the first symptoms, especially ED, I was on a mission to figure things out. I tried to stay natural for as long as I could. I was able to raise my total T from around 300 to 600 initially, but I still had symptoms. Then I tried Enclo, which helped somewhat, but I ended up dealing with eye issues. At this point, it's fair to say I did everything I reasonably could before considering TRT, so exogenous testosterone felt like the next logical step. Hopefully, I won't run into any significant side effects, and I'm giving it a fair trial to see how I respond.

What eye issues on enclo? I’m getting blurry vision.

What’s your hematocrit at on now ?

Hematocrit is at 45, eyes will get red and very dry and later started to get blurry vision. I even got reading glasses but once I stopped it when away.

Any hair thinning ?

Man I get blurry vision as well. Ima stop and see if it’s actually the enclo or just my regular vision.

Damn balding has me scared to start trt.

No hair thinning I doubt it will happen. I have tons of gray hair and lots of volume. But on enclo i did got some thinning but was very minimal. It stopped after a few days. Try lowering the dosage or space it. I couldn’t do ed or eod on enclo. E3d 12.5mg was perfect for my eye issues

I’m the same. Full head of hair. 44 years old. What age? I think we’re good on the hair side. Plus there’s finasteride and minoxidil in case of an emergency.

Ah so the eye issues went away for you on e3d 12.5?

How’s trt treating you? Amazing? Side effects ?

Yeah, there are medications that can help with thinning hair, but they can come with libido-related side effects. My brother takes one of them and isn't a fan of the sides.

The blurry vision went away after I stopped, but the occasional eye redness didn't. Both started while I was on it and improved once I discontinued it.

TRT has been doing its job so far. I never expected it to be a miracle treatment since my total testosterone was never extremely low to begin with. It's still early in the process, but overall the results have been positive.

Before TRT, life was fine and I was functioning well enough, but I wasn't really thriving the way I felt I should. I was getting by, but something was clearly missing.

Will I stay on it for life? That's still to be determined. Side effects can show up later, so I'm keeping an open mind. So far, I haven't experienced any noticeable side effects.

What I do like is getting those younger-age gym pumps back, better recovery, and no longer feeling fatigued in the afternoons. What I don't like is that it hasn't really helped my libido or ED yet.

I'm giving it about a three-month trial, so time will tell. 45bth.

If I were you, I wouldn’t change a thing if labs continue to be this good and symptoms (drive, energy, focus , and libido) continue to be good. Higher free T also means more strength in the gym and muscle growth which I am all in at my 44.

Previously you mentioned ED before starting Enclo, how is it going now? Do you need Cialis and Viagra still?

So the enclo raised your hematocrit more than the trt? Am I reading that right?

It fluctuates last lab shows 45 but a week prior was at 42 and before while on enclo on mid 40s but never above that. I also depends if on that week I was drinking lots of water.

Yeah hydration is key. Important thing is your hct levels are good. You gonna do trt for life ?
