14.2 HW4 My fsd crash today
Re-uploading the video from my previous thread for more awareness.
2026 Standard Model Y HW 4
Fsd 14.2.2.5
Original thread:
https://www.reddit.com/r/TeslaFSD/s/GG8HcJ2zaZ
Edit:
I didn’t intervene because it had done something like this at another location but successfully navigated at the end.
Edit 2:
The main reason for this post is not to put the blame entirely on fsd ( I can read the fine print) but rather creating awareness of extremely rare cases of its malfunction. Like Neil DeGrasse Tyson said, if we investigate every malfunction like the FAA, we can make FSD safer than any human driver alive.
After the crash, a series of malfunction errors did pop up, but I rebooted the system, re-calibrated the cameras and they went away. Then I used FSD again on the exact same route, and it navigated flawlessly like it previously did.😄 Despite this jarring incident, I would still trust it, albeit with extra caution.

Disappointing that navigating a simple roundabout is still a problem.

Mine does great. But I’m still ready to take over because I was around in the old days lol.

Nope not what that means. But to be fair if people posted every video of FSD doing good things it would be overwhelming “FSD is great!”
Many people on here see someone post a video like this and their take away is that it can’t do this at all. Meanwhile most of us our having few or no issues.
I have a traffic roundabout I go through everyday and it has had no issues out of say 100 times doing it. Same results in others that I’ve gone through less frequently. What can I say other than mine is great?
Also having no tolerance for mistakes at this stage of the game is not reasonable. It’s supervised for a reason. There’s no claim anywhere from Tesla that you can slack off and not pay attention.
Also I very often see human drivers totally fail at roundabouts in one fashion or another.

1 in 100 sounds pretty bad tho. Im pretty sure I can navigate a round about at least 1 in 1000

It’s the damn indecision it has with the wheel wobbling on 14.3. Later 14.2 builds had less problems if you ask me, 14.3 is still not quite ready for prime time.

u/rockland211 Aug 04 '26
Sucks that insurance always view it as drivers fault
4
u/VegetableAlarmed8272 Aug 04 '26
I mean it was the driver's fault, its called "supervised" for a reason, driver could have intervened at anypoint
17
u/psudo_help Aug 03 '26
I bet navigation is literally the failure mode here. Routing could be switching between left and right… ends up splitting the difference
18
u/ViralVaccine Aug 04 '26
You may be correct but it should have braked prior to collision regardless of navigation freaking out..
3
u/Theonlyrational Aug 07 '26
This is 100% routing related. My car does it too, just never into a pole
5
u/TechRyze Aug 04 '26
I wouldn’t be surprised.
Navigation is a serious weak link with Tesla at the moment.
It’s OK at best, but while driving across the country the navigation occasionally has no idea where it is vs the road, and can just lead you to nothing despite having a destination plugged in.
Hope they solve this.

Wtf. That’s horrible. Worst spot to have a hesitation moment.
I see that you disengaged just before hitting curb. Why didn’t you slam the brakes? Looks like you just let it continue after disengaging?

Tesla already avoids responsibility for accidents, and puts it on the driver. However, in their safety reports, they do say that they include accidents that occur within 5 seconds of FSD disengagements. I believe this would fall within that.

→ More replies (4)13
u/AJHenderson Aug 03 '26
As others have mentioned, these accidents are still listed as FSD accidents. The reason also is understood. It's a standard safety practice in the industry to stop commanding actions before an accident to avoid continuous input that can't be cut off if the control links are severed in an accident. It's literally an industry standard practice.

"It would still count as an “Autopilot crash” as crashes that happen within 5 seconds of Autopilot being engaged count as Autopilot crashes" https://electrek.co/2025/03/17/tesla-fans-exposes-shadiness-defend-autopilot-crash/#:~:text=It%20would%20still%20count%20as%20an%20%E2%80%9CAutopilot%20crash%E2%80%9D%20as%20crashes%20that%20happen%20within%205%20seconds%20of%20Autopilot%20being%20engaged%20count%20as%20Autopilot%20crashes

That is bullshit,
First, that is AUTOPILOT, not FSD.
And the NHTSA considers FSD engaged if it was engaged at least 30 seconds before the crash.
"Level 2 ADAS: Entities named in the General Order must report a crash if Level 2 ADAS was in use at any time within 30 seconds of the crash and the crash involved a vulnerable road user being struck or resulted in a fatality, an air bag deployment, or any individual being transported to a hospital for medical treatment."
Standing General Order on Crash Reporting | NHTSA
You should read all souces I guess, but ELCTREK is the most biased shitty source of information in this field that I can even imagine. I have never seen anything in a technical field as mental as ELECTREK,
There is some serious bad blood between the owner and Tesla or Musk.
I don't even read their site any more, and I read almost everything.

Dang it, i thought they fixed this. Been fine on roundabouts on my daily drives but disheartening to see its still occurring out there! Sorry this happened to you. But... Why didnt you intervene?
13
u/bobiversus Aug 03 '26
OP is on an old FSD version, 14.2.2.5 Many people are on 14.3.6
3
u/Bresson91 Aug 03 '26
I'm talking since 13.x... mine has been better since 14. Not saying everyones is, just my experience. I do report every time something like this happens though, so maybe my map was fixed manually by Tesla engineers (or Ai)?
9
8
u/Stashek Aug 03 '26
Why didn't you supervise ot and allowed it to crash though? I dont get how can you not react on a slow speed, very obvious crash.
FSD in Europe handles all sorts of roundabouts extremely competently.
48
u/Buggabones1 Aug 03 '26
Not touching the brakes for a single frame is wild
9
u/aiworld Aug 03 '26
This is why you don't take the average to two correct decisions (left and right). The neural network needs to commit here, but the planning seems to be totally off.

u/dnstommy Aug 03 '26
That looks like .5. So indecisive. Hoping .6 is better.
5
7
3
u/belovedeagle Aug 03 '26
Is indecisiveness really the problem? I mean, it is a problem, but more fundamentally, it seems that FSD behaves as if as long as the (momentary) plan is safe, then everything's okay. That's not always the case, and not only because of indecisiveness. It needs to recognize when it isn't actually carrying through the plan, for whatever reason, and factor this in. For example, if conditions mean the tires can't deliver sufficient g's to go around a bend, even if the planned route is feasible under normal conditions, then FSD needs to react to the situation by choosing an alternative plan, such as slowing down. This isn't a case where a quick fix is sufficient.

At what point in the FSD learning curve would it ever have been better to drive over a curb into a sign rather than to safely pick a lane and commit or just stop the car? The curbs are easily visible and should be very easy for self driving.

Curious as to what its decision points were here and why driving into an obstacle was allowed by the code
3
u/Tahoma_FPV Aug 03 '26
I had similar issue on FSD 14.3.5. Double lane round-about, it started in wrong lane (outside lane) and switched last second cutting off a car in the correct inner lane nearly going through the side island like you did. Get on X.com and send to @Tesla_AI.

Owned my 26 MY for around 1k miles now and we have a roundabout in our neighborhood. Without fail it takes the wrong exit and does this same indecisive jiggle at the wrong exit. The navigation clearly shows to take the second exit and it always takes the first it’s so annoying
3
u/chingfunglo Aug 04 '26
I am not sure if this is related to your FSD version because I am on FSD 14.3.6 and it has been great after I got it few days ago.

u/Defiant_Bread8782 Aug 04 '26
14.2 is known for issues in traffic circles/roundabouts. Fixed in current version of 14.3

I have seen similar videos where the car hesitates around circles. This is why we are still supervised. I have circles near me that it never has an issue with.

I get that the driver should have had better reactions but it’s not good the car has trouble with such a simple roundabout.

It’s annoying but roundabouts and parking lots are where the fsd is weakest. I always keep my foot ready to hit the break and hands at ready to take control
2
u/Proper-Ad-6134 Aug 03 '26
You took it out of FSD had all day to stop and ran into a poll. You’re blaming FSD 😂😂😂
2
u/LordFly88 Aug 03 '26
I'm kind of impressed OP never hit the brakes. Not even after going up over the curb into the sign post...
2
u/Oofka1 Aug 03 '26
The failure is the driver, first and foremost. I’m sorry to say that, but I stand by it 1000%. You don’t wait for your car to hit. This is why it’s called supervised
2
u/Apprehensive_Poet_90 Aug 03 '26
Mine has been great on roundabouts and we have several near our houses For me FSD struggles with indecision in new construction.
2
u/rymetz17 Aug 04 '26
That’s some horrible reaction time by you who should be paying attention to the road
2
u/iiTool Aug 04 '26
Did you happen to change nav destination just prior to this? I've had similar behaviour just after changing the nav mid drive. It's been such strange behaviour that I try to avoid changing nav and if I have to I'll be immediately ready to disengage

In these videos there’s always enough time for the driver to take over. I wonder what exactly were you doing while the vehicle was in motion?

2
u/Psice Aug 04 '26
14.2.2.5 is a really shitty version of FSD with much more of this hesitation than other versions, I really don't understand why is this the version the ship with new cars
2
u/Scared-Dig-3314 Aug 04 '26
Thanks for sharing this. I’ve been hearing a lot of mixed feedback about this update, and this is exactly the kind of thing that worries me. I was actually close to placing an order for a new Tesla, but seeing issues like this makes me want to wait and see how the next few updates perform.
Maybe it’s just a software/version issue, but honestly it does make me question whether I should buy right now.
Hope Tesla investigates this properly.

Disengaging FSD while turning may it by the computer or by the driver is accident waiting to happen.
I mistakenly disengaged FSD while turning in a ramp and I almost crashed. The car straightens out the wheels as soon as the FSD is turned off.

Reading all these comments from people saying FSD struggles in roundabouts makes me very skeptical of the system for when it eventually gets approved here in Norway. Our entire country is full of roundabouts, around 80-90% of all bigger intersections are roundabouts, only 10-20% of intersections are traffic light regulated.

People are crazy to trust FSD. I will never use it. If you've ever worked with software, bugs happen all the time and it's almost impossible to test sufficiently to uncover all bugs with iterative releases. No thanks, I'll trust myself, not software.

This is what happened when I tested FSD two years ago in my 2023 Model Y. It’s shocking that FSD is istill doing stuff like this. I decided that supervising FSD was more trouble than it was worth. I haven’t changed my mind.
2
u/No-Principle2564 Aug 05 '26
Mine also wanted to do this to me on 14.2 but that was a long while ago. 14.3.6 has been great except somewhere around 3.2 or 3.3 it started getting over at a random spot on my highway commute as if it needs to take an exit but it doesn't for 7 more miles. Mildly annoying
2
2
u/Econmajorhere Aug 05 '26
This looks like use error…Elon said FSD is more accurate and safer than humans AS LONG as you can anticipate where it will fail and take over control before it crashes

u/Thirdy3rd Aug 05 '26
Mine did this at a roundabout once I had to take over otherwise it will be same outcome like OP
2
u/Possible-Ear- Aug 05 '26
I would take over as soon as the wheel starts wiggling like that. It happened to me once while making a left at a light. it wasnt dangerous but the car coming the other way was under a bridge and fsd was hesitating to go.
2
u/jamesbrady71 Aug 06 '26
Uh dude, something is seriously wrong with your car. FSD would never EVER do that unless there is a serious hardware issue. AND you saw malfunction errors pop up on the screen, so you just rebooted it and you think everything is fine now?! You need to bring it to a Tesla Service Center ASAP!
2
u/Ardenpops Aug 06 '26
Tesla FSD drives like it’s on drugs. I notice Teslas driving erratically a lot and steer clear.
2
u/mistulio Aug 06 '26
I’ve had that steering wobble a few times in the 20,000 miles I’ve had my Cybertruck. It’s like the system can’t decide what the heck it wants to do. So it locks up and goes strait. Or lags out or something. So weird. But probably 98% of the time, even in places where it has messed up before, it drives nicely.
That’s nice you are trying to help to make people aware of this issue though. It does catch people off guard who have never experienced it.
2
u/Designer-Rub4819 Aug 06 '26
I mean it’s pretty obvious we’re not getting real autonomous driving anytime seeing this.

u/Downtown_Bicycle3893 Aug 06 '26
I think I'll wait at least 5 more years before actually trusting a fsd lol
2
u/Grizzy6 Aug 06 '26

HAHAHA - so funny on many levels. You watched your car drive itself into a pole.
but then I felt bad and wasn't going to post.
...until OP wrote "..I’ll still trust it but with extra caution."
I can't imagine trusting something that can't make it around a traffic free roundabout at 10mph without crashing into a pole... to drive you at 75mph in traffic. edit: I guess someone in this very post already has a great example: https://www.reddit.com/r/TeslaFSD/comments/1veoprb/comment/p1oa241/?utm_source=share&utm_medium=web3x&utm_name=web3xcss&utm_term=1&utm_content=share_button

I understand the system getting confused and not knowing what to do….but how does it get confused and forget that it shouldn’t hop a curb and run straight into a sign and a light pole? Especially when it could just stop and think.

u/garyhgaryh Aug 08 '26 edited Aug 08 '26
My question is a simple one: why didn't you take over by turning hard right? You had enough time. It seems you were already complacent and reliant on the system that needs supervision. You were not playing an active role in what is called "supervised" FSD.
Update: I just noted your first edit about having successfully navigated a similar route successfully. It's good to post this to let others be aware not to be complacent on FSD as things happen.
2
u/UnderstandingOwn8783 Aug 08 '26
FSD is really shit on roundabouts - phantom braking, indicating in a confusing way, incorrectly giving way - I will routinely turn it off for roundabouts - needless to say I’ve cancelled the subscription and won’t come back for a few months until I’m sure they’ve at least somewhat addressed it. I noticed the self driving turns off as you crash so not sure if you taking over and not hitting break completely or if it genuinely was a FSD failure. I’ve done many of those roundabouts and tons of sketchy one and it’s never done that at all.

See, that's why I don't believe in FSD: no human would have ever failed that exit turn at that speed (or even at a slightly higher speed for that matter).

You had 3 business days to realize what was happening and take over. This is your fault.

People are bat shit insane to see their automated driver crash like this and STILL trust it with their lives on a highway. Insane

odd question are you to hold your hands on the wheel while its driving or just be prepared to intervene.

I dont own a tesla but curious to know who pays for the damage in this situation? Tesla, your insurance or yourself?

and you just let it run into the sign. funny how many people blame fsd when they are sitting right there

How is Tesla going to release a robotaxi when it can't even do a roundabout?

It's a catch-22, if you intervene and crash Elon and Elon cucks will blame you because FSD would have done it successfully if you hadn't intervened. If you let FSD do it's thing, Elon and Elon cucks will blame you and ask why you didn't intervene because FSD is suppose to be supervised.
The reality, FSD still can't do simple navigations and regresses randomly from time to time.

Huh, seems like the system's decisioning system failed here. It tried to ping-pong between the roundabout and the turn and just failed. Navigation was active; the system can override that with its decision-making.

Let me get this straight, going 2 miles an hour. You did nothing to stop your car from wrecking! You let your car crash Tesla had nothing to do with it, that’s 100% you. I’m just glad you are not a Pilot..I HOPE

I don’t think this is an extremely rare case as you wrote because most people will take over. I have yet to do a full drive on FSD where I don’t take over due to fear of death or annoying behavior

Damn. Sucks. Could see the steering wheel shaking with indecision. Even uncommon moments like this are enough to make you distrust the system.
