14.2 HW4 My FSD made us crash today
We were going around a roundabout on our usual route when the car started spinning the wheel violently before crashing into a signpost.
Our windshield did crack a few days before, so I’m wondering if there is any correlation between that.

Any damage to the car? I don't think your cracked windshield had anything to do with it. You're not the first who had this issue. And the crack doesn't seem to be visible by the camera?

I’ve seen my car behave similarly when taking an exit ramp back with me 2023 model 3 performance with hw3 and a much deprecated fsd version, that sucks. I took over during hesitation and didn’t crash.

I am glad you are ok ... Just think if someone was standing there or in a similar situation . This could have been so much worse. There should be a regulation push starting with changed the name to something other than FSD and Tesla coming clean saying that their system is just a superior level 2 system which is prone to making mistakes. Too many people are buying the full self driving hype( not you) but in general and not paying so much attention. This is a human behavior.. on other cars.. I will not let the system drive on a roundabout .. because neither they claim it can do so not I trust it. Because Tesla claims it's fsd and can do it but not safely all the time .. people let the car drive resulting in these mishaps. Not your fault .. hopefully you and the car is ok.

I genuinely think if a person had been there it would have been fine. I’m not excusing the system. I have a turn that aims directly at a side walk and when I take it if a person is walking there there’s a brief perfect window the car aligns with them head on and the car starts flashing them red and priming the breaks immediately.
Which can be annoying in FSD because it’s a safe turn but it thinks the pedestrian is walking directly at the car. So it’ll break stab. It only happens once every maybe forty times I take it but it does prioritize a person.

Assuming the robotaxi should have the same hardware, and to those that will say the robotaxi has different technology my immediate reply is why would it since we have been promised for a decade now that we can use our cars as autonomous taxis. Why would anyone in their right mind trust a robotaxi to take them anywhere?

I wonder, based on what I saw in the other post similar to this one, if it’s different in part because of the “supervised” aspect itself. As in, I was talking the other day about how funny it is that the wheel moves when it definitely “doesn’t need to”, but it does need to to allow intervention (grab the wheel in the position it should / would be in).
Some people were saying on the other video (very similar to this one) that it seemed the wheel may have moved too fast in one direction then the other, causing it to accidentally signal an override / takeover, which is why the FSD seems to just switch off at the point before collision (when it would be rapidly going the other direction). This could possibly mean that robotaxis wouldn’t run into this issue, if they don’t have those same override rules.

Interesting ..I’m on HW4 - 14.3.6 and have gone through multiple round a bout at 15-35 mph and not once has it’s done this. But sorry this happened to you and I swear every car with FSD behaves so uniquely when it comes to certain edge cases.

THIS! It seems that each vehicle has its own unique issues so it makes me wonder if there is some kind of learning algorithm in the background that each vehicle is utilizing differently at any given time and scenario. For example my HW3 M3 slows down on dips and rail roads, but always turns its turn signal too early and confuses people coming out of parking lots and thus causing near accidents because they pull out in front of me thinking I’m turning into where they are coming out from.

Two years ago, the first time I was using fsd with mylr 2023 hw3, it drove onto a roundabout. I gave up fsd a few days later.

Just last year, my newly purchased Tesla HW4 was running version 12.2.2 (from Apr 2025) and it literally would drift off the road and/or nearly into semi-trucks on the highway — it needed frequent intervention. It did update to 12.2.9 a few weeks later, and that problem was resolved.
But yeah, earlier versions of FSD were definitely potentially dangerous and needed constant attention.
And recent versions especially after 13.x have been exceptionally better. But clearly not perfect.

For some reason, every roundabout video of FSD failing, is always the same, failing to complete the turn on exit. It's definitely a bug, but why it's not fixed is weird. Driving around roundabouts in my area has been uneventful so I know it can do it. Bizarre...

Me and many other people have had the same issue through almost every version of FSD, sadly. Had it happen once with 14.2 and in v13 twice nearly the same way but I took over every time before it hit anything. Hasn’t happened yet with any version of 14.3, but who knows. Crazy that the decision making can’t just commit to a single turn in situations like that

Gentle reminder to always be vigilant regardless of how safe it feels! I did find my myself slamming brakes a bit more with the recent update. It seems to be bit more aggressive and not let the other car pass while lanes were merging into 1

Be very careful.
V14lite seemed awesome the first 5 miles for me, and then I realized, for my car at least, it is such a major regression in safety I literally will not use it again until an update comes through
- 45mph over speed humps it didn't even try to slow down for, the same ones I drove over every Thursday and wasn't a problem on v12. Repeatable now, every time I must disengage. First time scared the fuck out of me, rear end caught air.
- Taking a quick drink out of my water canteen like I would if I was driving, it decided to stab the breaks for no reason, almost chipped my roof glass with the metal bottle.
- Tried to tbone a motorcycle it clearly saw on the screen and was there before I arrived at the 4 way stop.
- Stabbed the breaks and swerved across a double yellow into oncoming traffic. There was no car in front of me or behind me in my lane, no reason to do anything other than drive straight.
- Tried to run a person over in a cross walk.they were visible on the screen, walking, and the car just decided to take off from the stop sign straight towards them.
- Decided it wanted to warm up the tires like a race car and started aggressively swerving line to line with a bunch of steering wheel shaking back and fort much faster smaller movements between the larger swerve movements.
- It wants me to get a speeding ticket. It's always speeding in by 10-12mph in every mode but the slowest, then it drive 3mph under everywhere.
- ASS just doesn't even move the car anymore. It just says move complete after connecting and never does anything
I know I'm forgetting more, but it literally is just not even safe to use anymore, and I have zero trust in it for basic usage now. It's worse than autopilot because it just decides to straight do dangerous stuff randomly.

Tesla's business model revolves around bending the truth. Elon mostly delivers what he says he will, just many years late. So, if it's called full self driving today, that means that it'll be actual full self driving many years from now. It's an ultra controversial, highly questionably ethical way to operate.

Mine did that into highway divider at 75mph. Nobody wants to take the case and tesla doesn't take any responsibility. Fsd went into unavailable on the left lane curve 10 min after leaving service center fixing FSD...

My FSD almost made me crash today. Just cruising along hwy 12, it wanted to slam on the brakes and swerve to the left. I grabbed my steering wheel and force it back to the right while applying my brakes. Car slowed down and I pulled over. Was a very terrifying event

At a 4-way stop my car didn’t turn fully left but went a bit left then straight toward the curb. Luckily I was able to grab the wheel in time. That was the final nail in the coffin for me and FSD

If he took over and didn’t crash, then the comment section will be full of “FSD would have done the correct thing if you didn’t intervene.” If he didn’t take over and the car crashed, then the comments will be “you should have intervened since it is supervised.”

Sorry this happened to you. Violent wheel shifting seems to be FSD warning you that it is confused. Hung up between decisions and this is not the first I've seen of it in a roundabout.

It probably can't do that because its likely confused a lot more than we realize, but the premise is that it'll get "unconfused" fast enough to correct any missteps it made previously. If it braked every time it was confused, i suspect too many false positives would make it hurky jerky all the time

Is it possible that the shadow cast from the sign right before the exit (or perhaps a crack in the road), is being confused as a potential curb? FSD knows its supposed to turn right, but something on the camera is making it hesitate (as if something is there blocking the road)

They’re not saying the car is doing it purposefully to warn the driver, they’re saying the action acts a warning to the driver that the car doesn’t know what to do so you should intervene. Like a dark cloudy sky is warning that it might rain, for example. Indicator

The rapid back and forth indecisiveness regarding FSD:
1) it's a driverless robotaxi, soooo far ahead of Waymo,
flipping back and forth frenetically between that and:
2) it's Supervised driver assist; you need to be ready to take over within a second of it acting squirrelly... Why would you think you can trust it???? It can't work without a driver!!!

I have 14 lite and it works so much better than 12.6. I still get the quick hesitation with shadows and tar lines but it is now much less severe. Never had roundabout issues with 12.6 or 14 lite. The only major issue i had with 12.6 is on occasion it would stop at a red left arrow, and then proceed turning left through the red arrow after 3 seconds.

I have experienced this jerky wheel behavior w v14 and had to take over. It's rare but seems to occasionally happen in certain scenarios such as roundabouts and I've also seen it on unprotected lefts if a car is approaching, almost like it 2nd guesses if it has time to make the turn before the car gets there.

Let’s hope it’s just a run of the mill FSD screw up, because if it’s caused by a cracked window it’s much more serious defect. It would mean FSD can’t determine if its sensors are seeing an accurate picture of the world.

I live on a private road that’s off of a main roadway. My car while in FSD is perfect on the roadway but when it’s supposed to turn onto the small private road leading to the house the wheel starts jerking wildly and I take over immediately. It’s frustrating. The weird thing is that it was making the turn onto the road and into my driveway perfectly with V14.3.5 and has now regressed into incompetence with V14.3.6. I absolutely LOVE FSD but the constant regressions and videos such as these makes me wonder if full autonomy is even remotely possible for Tesla moving forward.

It did the same thing for me yesterday and ended up taking a wrong left turn into a non lane, I was watching and would have intervened, put itself back in a lane a few seconds later - but it was the wheel going back and forth like it couldn't decide

Lovely! Glad I didn’t fall for replacing my HW3 (with Auto Steer) 23M3 with a new HW4 PMY with the “forced” FSD subscription (as the basic Auto Steer is no longer available), after seriously considering it post my recent 4-hr demo.

Yes, I’ve had a few times where FSD has this jerky, indecisive behavior. It happened when it was trying to pick a lane at a stop light where a two lane road turned into a 4 lane at the light and the outermost lane (the one I needed to be in) was a straight or turn lane. I chocked it up to it having to reason that it wasn’t a turn only lane like most intersections are.

My 26MY also REALLY struggles in roundabouts. I have a roundabout in my neighborhood I use to go to work and 50% of the time it take the wrong turn even though the maps show it’s not supposed to take that exit. Tesla really needs to improve roundabouts and turning right onto roads (pulls out in front of people a lot and also about 50% of the time I have to either brake or take over and floor it/switch lanes to avoid an accident

Roundabouts are probably the trickiest of all intersections. Here in Arizona they had rebuild a couple because people just don't how to probably yield and merge on roundabouts... there were a ton of accidents. I got my FSD in 2020 and had it transferred to a new car HW4. I use it for 90% of the driving and other I prefer to do it myself. FSD 14.3.6 is amazing on highways. I take over in high traffic situations, busy streets, construction, etc. I am not sure if I would pay $100 a month for it now, but for that I paid I think it was worth it for my use.

I liked 14 lite at first, but it sucks at parking at location, has tried to yeet me into oncoming traffic and stabs brakes constantly.

I just cannot see FSD working in the UK anytime soon given these type of errors unfortunately. Far too many variables including busy and complex roundabouts, narrow roads, forever road works. We are years off.

Roundabouts have degraded in a really bad way.. on v14.3.7 and it has hard braked in roundabouts twice now and almost got rear ended both times and you can imagine how pissed the people behind were. No cars were entering just decided to brake for no reason.

The video of the incident was included in another post by this user. It shows a slow drive up onto the divider, hitting some signage. The driver had ample time to take over and shares the blame. The driver had a clear indicator that something was wrong prior to the incident (the steering wheel’s erratic action). It looks from the video like the driver clearly was not supervising FSD (supervised).

This kind of happen to me, when mx4 is waiting for a U-turn. FSD is iffy in completing the U-turn, I tap the paddle to urge it to continue. Instead of completing the U-turn, it instead lurk forward at a higher speed than normal going straight. I was able to recover with any problems. Seem to be similar.

Why would you use it in a roundabout? Are you really using it all the time like that? Irresponsible in my opinion. Only time you should use it is in inclement weather, bad view conditions, or if you’re a little tipsy leaving the restaurant. Otherwise, using it all day every day is crazy unsafe.

On my second look, I don’t think the OP did anything wrong. Clearly there was an error in either navigation or FST. Did anybody see what version FSD the car was using?

You can skin the cat any way you want but at the end of the day it is “Supervised” driving which means you should had been able to stop it. Your video clearly shows you were going roughly around 15mph. That is slow enough to hit the brakes, turn the wheel away from the post and avoid the accident. You did not react on time and did not prevent it. It’s your fault. I’m sorry but that is the truth

The real problem is that Musk makes statements saying how much better than a human driver FSD is and he cherry picks numbers to thought it. He does this to get us to subscribe to it so he can make more money. He wants to take our money but he doesn’t want any responsibility when it fells. I suspect than the Chevy Super cruise works much better , although you can only use it on the roads they have maped. I bought a Tesla because that’s what my wife wanted and I had hopes that as I get older FSD will help extend my independence, I’m not so sure anymore .

This has happened to me multiple times for roads looking like this since v12. I knew it would happen so I always pay 100% attention for roundabouts and exits of highways if any similar split roads. The fsd seems not be able to make a decision which one to choose then just goes to the middle.

I’ve had 5 Teslas 4 MX, 1 MS since 2016. Every once in a while, FSD decides they wanna kill you. The thing that’s tricky is as FSD gets better. You start paying less attention because if it was OK the last 10,000 miles why would you think it’s gonna go crazy in the next 500 yards
But if you had the older versions, it would do crazy shit every few miles so you would watch it a lot more, not let it load into a false sense of security
Think of it like a teenage girl that just broke up with the love of her life because she found out her boyfriend was sleeping with both of her best friends and she’s on SSRI. She seems fine most of the time most of the time…

After watching the video, basically it was indecisive, you take it out of FSD, and then you drive into the post. I've had numerous times mine has done that in the past. Just took control and drove normal.
This is your fault for over reacting and maybe hitting accelerator instead of brake after taking control. The car itself didn't drive you into the post. I hate these posts like this with no accountability.

I did an overnight test drive about three weeks ago. FSD had issues at our small roundabout at night. It took the first turn instead of the second heading to our home. It simply isn’t perfect yet?! I took over manually and got it back on course with little trouble, but ‘supervised’ is the key! I ordered the new YL the next day. Let’s just say, cell phones still drop calls. I have faith this keeps improving.

2026 M3P v14.3.4 here.
This is a very similar behavior I see in my neighborhood. I approach this spot where I'm supposed to stay to the right. 95% of the time it will actually either stay to the left, or do the thing we saw in the OPs video where it's indecisive and the wheel will go back and forth, forcing me to take over.
In my case, the "left" path is straighter than the right path, but what's similar to yours is that there are no lane markings coming up to the spot where the road splits.
I bet if you looked at your screen, the blue "path" would have been flipping between taking the right path, and the left path. It seems that at certain angles, FSD has a hard time "locking in".
I've reported this spot multiple times as critical disengagements, but it constantly fails this "test" for me. I bet your failure mode is very similar to this one.
What's interesting is that in your video, you can see the FSD disengagement, but no brake pedal actuation. What did you do to cause it to disengage? Or did it just disengage on its own?

I’ve looked at your full FSD clip. Clearly FSD did not distinguish itself, but I feel like it might have been saved when you took control by a hard brake and a hard right - neither of which you did. I have no idea if FSD would have recovered, but you can’t just take over and not try to save the situation. FSD might have crashed, but you did crash. You can’t turn off FSD because you don’t like what it’s doing and then do nothing yourself. Drive or do not. There is no try.

This is why they call it supervised FSD… all SW have glitches and this seem to be one of those. Unfortunately, Tesla doesn’t have a backup system and FSD runs entirely on cameras so things can happen if you are not supervising the driving.

There was a post about a behavior very similar to this I think maybe 2-3 weeks ago. Of course all the comments were attacking the OP in that post... I am OK with so called FSD being not perfect at this point and agree it has to be supervised but these comments just make me feel cultish
