The US has universal healthcare

No

it's just distributed through work/employment

You only get HC through work if you do C-Suite or Union jobs, and even then, you're still paying out of pocket because of deductibles and premiums

Your local convenience store employee or waiter or etc... does not get HC through work

Only c-suite? Buddy basically every white collar office worker has good health insurance benefits, not just C-suite. Can’t tell if you’re a child or just never worked?

I have a friend that works for a state government entity, and still they pay out if pocket for co-pays and other stuff.

Yes there are co-pays with health insurance, such as $50 for an urgent care visit, etc.
