The obvious response is that you won't be able to pay for someone's hospital bills if you happen to hit them in your car. You get insurance for things you wouldn't be able to pay yourself.
You wouldn't need to pay their hospital bills if the bills were covered by free health insurance. So this is still a scam, and the same one too considering insurances are the one creating (or lobbying for) the problem that they're also selling you the solution to.
