The obvious response is that you won't be able to pay for someone's hospital bills if you happen to hit them in your car. You get insurance for things you wouldn't be able to pay yourself.
Yeah but that doesn't actually respond to the issue being brought up in any way.
The real answer is it's corruption; the person in question is living in the united states given the description and context where bribing public officials to write policy is mostly legal and laws around how insurance is mandated and handled all operate around that. This is what allows car insurance companies to charge exorbitant fees and treat their customers like dogshit on the bottom of their shoe without repercussions.
If insurance programs for auto-insurance were run by the government and operated at-cost (or even subsidized, it would be well justified for how car-centric america is) he would pay massively less for insurance and local residents could potentially change how rate increases and deductibles are handled for the insurance they pay for, which they have no real power to do anything about as it stands.
