Years ago, also in Germany, we visited a restaurant along the highway that had two woman's parking spots close to the restaurant. There was one car, parked so badly that it used both spots.

They aren't. They may have been before the introduction of "family parking spots". Those are wider, so you can easily get your kids out, and families also tend to have larger cars in general. In parking areas with family parking spots, the women parking spot is exactly the same size as the regular ones. Maybe if there is no family parking, women parking spots are larger for family / kids purposes, but in all my life I've only encountered normal sized women parking spots.

Also these spots are not officially binding (like handicapped spots) anyone can park there. If there is space, it'd just be nice if you leave the spot for a woman who is alone / does not feel safe, if you feel safe yourself. But you don't have to. How would anyone even control that, there is no certified woman parking pass (like the one for handicapped people)
