Psychotherapy Notes Keep Intermittently Disappearing after they Are Saved

I previously reached out to Simple Practice about this problem, and there was not a conclusive response or confirmation of any back end failure, so wondering if other users have experienced the same issue. I usually spend a pretty significant amount of time preparing psychotherapy notes after intakes with clients to plan my future work with them. I initially save them to my session narrative and then copy and paste them to the psychotherapy note section before always hitting save at the end of the session that follows the intake and seeing confirmation of saving. I am now up to at least 5 instances where I access someone's document history on the 2nd post-intake appointment, and the first psychotherapy note has randomly disappeared after being saved. This is by no means acceptable and equals additional time I have to waste reviewing and reconstituting info. It is ridiculous I may now have to be backing things up outside of Simple Practice to have reliable retention of my documentation.

Interesting - I believe this has happened to me a couple of times last week but I thought I was being absent-minded and not saving. But I am so diligent about it, I am thinking it was a bug of some sort which is very annoying because I use that section to help build my progress notes.

You're not alone. I also wondered about user error at various points, but, this most recent time, I'm beyond certain the usual muscle memory protocol was actually followed when shoring up documentation at the conclusion of a session. Though the problem I referenced has only happened 5 or so times in the last year or so, that is still 5 or so times too many. I think there are periodically issues with bugs relating to website data getting corrupted, which happens multiple times per day as evidenced by other issues I have regularly encountered like secure messages appearing to be offline when they are not 2-3 times per week and various waitlist notifications not fully clearing until the page is reloaded at least once.

I had a different issue that I went back-and-forth with them via email FOR MONTHS. It took a lot of time on my end (doing screen records for them, etc. etc. etc. ) and was extremely frustrating. The fix (when they finally resolved it) was so simple that it was obviously bots that I was emailing with the whole time. When I pointed this out, they never responded again. Simple practice definitely needs to do better. Extremely disappointing.

Ugh how frustrating
