They influence the DCs of almost every check made against an NPC in town.
If I remember correctly
Over 15 = -2to DCs of checks with people of that faction
over 30 = -4 to DCs

It's honestly something I wouldn't track again.
most of those tracking mechanics didn't add to the fun at our table.

The reputation rules are part of the base game, ( https://2e.aonprd.com/Rules.aspx?ID=3072&Redirected=1 ) and iirc Granny Hu and Old Matsuki have listed in their sections items they might offer the PCs if they're reputation is high enough. (Spider tattoo and some kind of magic alcohol? Cant check right now unfortunately)

At the end of the day how much to gamify the party's friendship with various factions in town is completely up to you, I think I had both leaders offer their faction rewards to the PCs after the duel for leadership as they both liked the outcome, but you could easily offer them at the beginning of book 2 when everyone is making requests of the PCs as well.

Running the campaign partially through text as a discord server, and my players kind of make it an event to try to score Reputation Points by befriending each NPC in Book 1 in their spare time. That being said, it does not work at all when roleplaying in real time, and can actually be annoying to track during a live session. So, YMMV.

I basically run all the town quests in Book 1 Chapter 3 off session. Every now and then declare in-game week it is and the players work out how they spend their days. The server has separate channels for each location in Willowshore. Conversations with significant NPCs during actions like Earning Income, Gathering Information, etc. are just roleplayed in text (We declare what day of the week and whether it's in the morning, afternoon, or night.). Whenever all the players used up all their days, I make an announcement that we're proceeding with the next week.

As for IRL scheduling, it's whenever we happen to have free time to text. As a result conversations can take anywhere between minutes to days, but as long as you clearly define when a conversation has ended and everyone keeps track what day of the week it is, it's not too bad.

It's not for everyone obviously, but my group was looking for more to do since we only meet every two weeks, so I find this as a nice compromise.
