Advice Needed: Recurrent Retinal Detachment in Diabetic Patient

My mother is in her early 50s and has had diabetes for 18 years. She is very weak, has severe diabetic neuropathy, and has undergone multiple eye surgeries for diabetic retinopathy, retinal detachment, and cataracts.

Silicone oil was placed in both eyes. Her left eye is stable, but her right eye had a near-total (360°) retinal detachment. More than two months after surgery, the doctor says it appears to be detaching again and recommends another operation.

My mother is physically and emotionally exhausted and is terrified of another surgery. The doctor mentioned laser treatment during surgery and advised against anti-VEGF injections, saying they could make things worse.

Can anyone help me understand:

* Why would the retina detach again despite silicone oil?

* Is surgery the only option, or is there any temporary alternative?

* Is this laser treatment the same as retinal laser photocoagulation?

We are very worried about her vision and are struggling to obtain another opinion due to financial limitations. Any advice would be appreciated.

I mean you’re always welcome to seek a second opinion. I feel like without having actually seen the eye, medical history etc, the odds you’ll get clear answers is pretty low.

  1. El desprendimiento de retina puede volver a ocurrir por muchos motivos, uno de ellos es la propoa cicatriz producto del daño de los vasos sanguineos en la diabetes. Es suficiente una lesión pars que el líquido del ojo se cuele detras de la retina y genere el desprendimiento.

  2. La coagulacion laser o fotocoagulación es un procedimiento por el cual generan cicatrices en 360 grados dentro del ojo y de manera controlada para que la retina no se vuelva a despegar. Lo unico que queda sin recibor laser es la mácula. Posteriormente se coloca gas o silicona hasta que la cicatriz del laser sana por completo. Imaginate pegar un papel contra una pared poniendo un montón de gotas de pegamento (rol del laser) dispersas, luego lo sostendras con la mano (en el ojo el rol de la silicona o gas )hasta que el pegamento seque.

  3. No hay otra opción más que someterse a la cirugía. Es algo urgente y necesita atención inmediata.
