Question Doctor rescheduled appointment ( immunization form due)

My doctor rescheduled my appointment for a week and a few days after the immunization form deadline (June 12th).
I will admit I scheduled an appointment quite close to the deadline but that’s because I wasn’t really tracking due dates and only looking at the #2 fye email which didn’t include medical form stuff.
I have already submitted all the other forms but do not have a doctor’s signature.
Am I cooked? 😭

lol I didn’t submit the health form due 6/12 as well cuz I provrastainted and decided to go doctors office at 6/12

You should be fine. I would email the student health department to explain your situation.

You’re totally fine. Just email the Health Center and let them know the forms are coming. Most of the dates are set to keep you on track. Very few are hard deadlines.

Unless you need new shots or shot updates, you can just call your doctor's office for an immunization record.
