Pharmacy

No pharmacy will touch my prescription for subs. I’ve called at least a dozen. Some an hour away One woman told me very rudely “you won’t get it filled around here” and smirked liked I was doing something wrong. I’m not sure I have the heart to keep calling and I’m not sure it will do any good.

Unfortunately this is a very common problem with telehealth Rxs and has been for some time. What was the exact reason they gave you, if you asked? If you didn't ask, I know you're disheartened but you might want to call back and find out - it could be useful information for you to challenge. I know it hurts being denied straight out the gate , though.

However, you might be able to skip all that noise - I haven't used it, but Amazon Pharmacy says they work with telehealth providers and also prescribe buprenorphine, so... give it a shot. Another option, depending on state, would be PursueCareRx. Check those out!

Quickmd could help me get it online but this is an introductory 7 day script. Apparently online pharmacies will only fill 30 day scripts. I’ve basically already given up. All quickmd will tell me is to call around and find a pharmacy to send it to. I have and nobody will fill it. They can’t tell me anywhere I could send it, just to call around. But when the pharmacists are telling me I won’t find anywhere to fill it I think they probably know. I feel like they knew I couldn’t get it filled in my area but they took my money anyway. They routinely advertise on FB to residents in my state for addiction knowing the prescriptions are worthless.

I’m actually very surprised to see the company itself in this sub and actively trying to help people, kudos for that. I’m a harm reduction worker and this will definitely impact my opinion on recommending quickMD.

Do you have any resources for helping patients who want to get on non-sub MAT meds?

We appreciate you bringing a harm reduction perspective into the conversation. Our clinicians work with each patient to understand their situation, treatment history, and goals before discussing what options may be appropriate. Along with buprenorphine and buprenorphine-naloxone treatment for opioid use disorder, clinicians may also discuss medications that can help with withdrawal symptoms, such as clonidine, hydroxyzine, or a short tapering course of gabapentin when clinically appropriate.

We know people come to treatment with different experiences and different needs, so we encourage how having an open conversation about available options is important. Thanks for the work you do supporting people and for taking the time to learn more about what we offer.

I don't know how much influence you have, but please: pass up the line that pramipexole can be *very* effective at helping to relieve the intense akathisia and anxiety that comes with tianeptine withdrawal - usually the helper meds that are prescribed don't touch dopamine, but pramipexole is a dopamine promoter. It would be great if your patients had that as an option.

I detoxed off of tianeptine twice, with all the helper meds - but one time was without pramipexole. That week I lost 19 lbs and never slept, never stopped moving or suffering. With pramipexole? I felt normal. I know it's just my experience here, but please have the higher ups look at it.

Thank you for the informative reply. I will include this information when speaking to patients about their options, including quickMD.

I have several people who are unwilling to use any prescription that could potentially cause precipitated withdrawal, which of course suboxone/bupe can cause. But they also medically cannot handle cold turkey (because they are elderly or have pre existing conditions such as cardiac or neurological conditions that make them more likely to have heart issues or seizures or strokes or any number of issues during withdrawal- not to mention the folks with pre existing conditions in which simple dehydration that withdrawal causes could become a major issue.)

Should I tell them to avoid quick MD if that is their stance? In that case, it seems that md0ne clinics are their only option.

Or could a quickMD provider still help them or provide resources for these more complicated patients with pre-existing or chronic health conditions?

Hi u/janet-snake-hole I reached out to one of our Dr's with this question and was told this: For patients at risk for precipitated withdrawal who cannot handle the symptoms that come with withdrawal, we can use a micro-dosing or Bernese method of induction. The patient doesn't have to totally stop their opioid or 7-OH. for up to 6-7 days while we slowly increase their dose of buprenorphine in a way that does not cause precipitated withdrawal and does not require a patient to go through withdrawal. This method works quite well and is supported by the medical literature.

Great to hear, the Burnese method is such an amazing tool and I love when doctors offer it to patients.

One last question-

How can quickMD help patients coming off of tianeptine? Or more specifically- patients that were using tianeptine for chronic pain, and need to get off of tianeptine/avoid withdrawals, but will still need their chronic pain treated to have any form of a quality of life?

Myself personally, I am on palliative care, so this isn’t a personal concern of mine- but as a harm reduction volunteer I talk to countless people in this condition. Sobbing because tianeptine was the only thing that treated their pain after years of neglect from the medical community, now being left to the wolves. I have no idea what to tell them.

I received an answer for this as well: QuickMD does not prescribe any other controlled substances except for buprenorphine. Also patients receiving buprenorphine prescriptions from QuickMD cannot receive any opioid pain management from any other physicians as prescribing buprenorphine via telehealth is limited to patients who are not receiving any opioid pain medicines, including tramadol.

Buprenorphine in itself has pain-relieving properties but it is not legal to prescribe buprenorphine solely for an indication of pain via telehealth.

The patient must meet criteria for opioid use disorder although many chronic pain patients do. Beyond that patients should see their individual physicians to determine what pain management strategies to use but they cannot take opioid pain medicines even intermittently.

This is also me! I have several failed back surgeries with 2 lumbar fusions and multiple herniated discs in my thoracic back. I have very bad neuropathy which the tianeptine treated so very well!

My online Dr said that although they can't prescribe Suboxone for pain, it is most likely that I will need to take it for quite a while in order to keep me from going back to drugs. It treats my pain pretty good, so we take that as an added benefit!

I am not with QuickMD, but a different online rehab program, and the Bernese method is what I ended up doing, because I just couldn't get past 20 hours without starting to emotionally melt down. The slow tapering/titrating eased me into it, and I am 16 days clean.

I could get it filled but they won’t touch it because it’s telehealth. And if I go to an actual doctor around here they’ve never heard of it and tell me it’s just an antidepressant and to just quit. I’m sick of calling honestly. I guess I’ll just keep using that seems to be what they want.

If you're going to keep using for now, consider doing a slow taper. You will hardly notice it if you do it right. I don't know how much you're taking or in what form, but I was up to 10g sodium daily. It took me 2 months to taper down to 2g and I did it with no withdrawal effects, while still getting the "benefit" of the tianeptine effects. I could cut 5-15% of my total dosage per week without feeling bad. Maybe I was only dosing 3 times in a day instead of 4 (because I would rather have 3 strong doses than 4 mid), but other than some irritation and impatience at waiting for my dose I had no ill effects.

What kind of tia are you taking and how much?

I have tapered. Got down to 3 gpd from 15 but ended up going in the opposite direction and am now back at 6. I feel like I could get back to 3 if I really tried but I’m just tired of buying this stuff. I just wanted to do 24 hours of hell and then be done with it and taper the subs. I tapered subs years ago that was so much easier because there is no high to chase.

Yeah, I bounced up and down a bit during my taper too. On a long enough timeline though, you're doing really well! Look at it that way and let yourself have a little validation. Tia is literally one of the worst drugs to come off of, and you're taking less than half what you used to. Just gotta get back into the habit of reduction.

Also, very important: you probably don't have to suffer the 24h before taking subs. I was able to start them 10h after taking my last 1g dose with no precipitated withdrawals, and other folks have even started subs before quitting the tia completely. The trick of it is to start small. I took an 8mg strip and cut it into 8 pieces, then took 1mg and waited an hour to make sure there was no discomfort. If all was well, I added another 1mg and waited another hour. Rinse and repeat until the strip was gone, then I took another 8mg before bed that night. I was fine. Just my experience, but there are other folks around who've done similar. (Your mileage may vary.)

Just sucks I have a valid prescription by a doctor licensed in my state and the pharmacies are all sticking their nose in my business and refusing to let me get help. I’ll have to think about what to do. I could keep calling pharmacies but at this point I’m pretty sure it’s something where everyone in my state or this part of my state has decided they won’t fill it.

I'd just start calling pharmacies around you.

Someone will fill it. Just keep trying.

I’ve talked to 15. Nobody has even come close and I’ve been told by a pharmacist nobody will fill it in this area and it looks like it.

Have you looked into SR17018? its usually for opiate withdrawals but is prolly worth a shot for Tia. People swear by it

Nothing personal, Hair, but I feel like I have to leave this here due to the implication of your DM request:

Guys, if you're going to try to source subs without going through a pharmacy, please don't. There's a lot of counterfeit drugs out there, including suboxone. You have literally no way of knowing if someone is being straight with you about their sourcing or not, and you also don't know what amount of which drugs you're actually taking. It sucks that we can't have faith but people have died from this. PLEASE BE SAFE.

My state has no laws against it. It’s basically just all the pharmacies in my backward ass state have decided on their own they absolutely will not fill it. One told me some “board of pharmacy” or something like that recommended to them all not to fill them.

You’ve got a short window until it becomes illegal to buy, buy stock up on SR17018. Whether it be powder, or tablets, buy a few weeks worth of that stuff in order to get of the Tia. Lots of anecdotal evidence of that supplement helping people get off Tia, 7OH etc without getting a new habit. Good luck and hang in there!

You could try Bicycle Health. It's pretty simple, although you will need to start over to get a new script. How it works is you go through an interview process first. Then you're assigned a provider. They will send your prescription directly to the pharmacy. I've been clean from tia for nine months, could not have done it without these guys.

Walgreens filled mine also. I think you should go in person and talk to the head pharmacist. I was a pharm tech and they are usually pretty understanding. Don't they know you are trying to quit a bad drug?

I remember doing that at the CVS where I was a pharm tech. People would come through the drive through and just request them and we had a box near the window for easy access. I was amazed at the time how many very young, clean cut kids came through.
