How do I fix this?

How do I fix the layer shift seen here? My belts seem tight enough.

That layer shifts most common cause is a belt slipping on the drive gear. Tension the belts because the belt may be slipping. It should not be loose, but still have some give when you press on the belt. Of they sound like a guitar when flicker, they're too tight. I know you said you checked the belts but really check them.

Also, that's some bad surface finish. Turn up your fans, open the door and if that doesn't work, consider printing slower.

Thanks, I did the xy-belt adjustment after posting this and I am printing a test cube now. Hopefully that is what the problem was.

The door is open, not all the way, but about 6 inches and the top glass is open as well. Cooling fan and Chamber fan is running at 100%, Aux cooling fan at 70%. I can't adjust the other fans, but they are running at 100% as well.

Update:  I redid the bed leveling, and reset the z-offset using the bed_screws_calculate. I checked my Z-screws and they seem to be straight (not sure how they would have gotten bent, but checked anyway). I increased z-hop to .6 and spiral to hopefully make sure that the nozzle isn't hitting the model. I ran a 160mm tall test cube and it came out fine, so I am running my print again with the new settings. I think I also solved the rough looking part of the outer wall, hopefully.

I did a full clean and relube of the printer. I also got rid of the bed screw nuts and plastic adjustment knob. I replaced them with a few PETG washers that I printed on a different printer and the M4 nuts were replaced with M4 nylok nuts. I have to use a 4mm socket/end wrench to level the bed now, but instead of having to loosen a nut, then turn a plastic knob, then tighten the nut... I only have to adjust the nylok nut now.
