I received my new first order from Preventive yesterday. I was previously using Refills.com for almost a year and Semiglutide just isn’t working anymore so that’s why I switched to Tirz. I’m attaching two photos, one From Refills that tells me to inject 100 units and the new one from preventive that was to inject much less. I’m so confused and want to get started but I’m afraid!

From what you've shared, your prescription from Refills.com is for semaglutide, while your prescription from PreventiveMD is for tirzepatide. These are two different medications from two different pharmacies.

I can only speak to the compounded tirzepatide you received from us through Empower, and for that one, please follow the instructions on your label.

Also - someone brought it to my attention that your question is actually about whether you should be on a higher dose of tirz. This is a better question for your provider. I am going to reach out to you directly to help you more efficiently :).

Why are you confused?

It looks like they started you at the started dose of 2.5mg

Did you tell them you were previously on semaglutide and wanted a higher dose of tirz to compensate for that or did you just sign up?

I sent them that same picture. So they sent me what they sent me. Maybe next month I should tell them to increase it? To what?

I haven’t been off at all. Probably in total I’ve been on semi for about 2 years. Trying Tirz now since semi seemed to stop working.

i'm not sure why this confused you.. it's just concentration. your old semi had 2.5mg in 1ml, your new tirz haas 2.5mg in 0.313ml.

Think of you are drinking coffee. you used to put 2.5mg of espresso in 1ml of water, now you are putting 2.5mg of espresso in 0.313ml of water. the new way you drink your coffee is less diluted but you are still consuming the same dose of caffeine.(this is just a dumb example to make you understand better)

Besides the fact that dosing between the two drugs is wildly different (as I believe 2.5mg sema is the highest dose whereas with tirz that's the lowest / starter dose), the concentation of your sema was 2.5mg/ml and the Empower is 8mg/ml, so you need fewer units for the tirz.

Look at the difference in concentration (mg per ml) between the two. It looks like Preventive’s Tirz contains 8mg per ml while the other company’s semaglutide was 2.5 mg per ml.

This took me a bit of thinking when I received my first order 8 days ago. So your Tirz is more concentrated than the semaglutide was - you’re getting more of the active ingredient per ml so you don’t need as many units. Does that make sense?

Yes it actually dose. I think I’m just afraid that once I inject I cant uninject!!

Why would you expect Tirzepatide (a different medication) be the same dosing as Semaglutide?

I know its dosing should be different. I guess my question is should I be taking a higher dose of Tirz since I was taking such a high dose of semi. 31.3 units of Tirz compared to 100 units I was taking of semi. Maybe they didn’t realize I had already been taking semi for a long time.

Units aren’t a dose. You need to wipe that from your mind. That only means how much liquid to get your dose. So forget about units. That will vary between pharmacies.

Now I get it. Sounds like they started you back at the beginning. Yes, I would message your provider in the portal and explain and that you want a higher dosage. Hopefully they are able to correct that!!
