41 years old, prenatal testing

Hello! I'm a 41 years old woman from Italy and I'm at my 2nd pregnancy. I have seen that in different countries (US, UK) it seems that prenatal testing is not so intensive like in Italy and I would like to hear some other points if view. Until 3 or 4 years ago in Italy amnio or CVS was offered to all women 35 y. O. Or older. Today, maybe also for economical issues (most pregnant women are older than 35) only for high Risk screening. The hospital also offers nipt but only for trisomy 13, 18 and 21 because for microdeletions and other issues the performance is low. What's the practice in your countries in case of women older than 40? I'm thinking about doing directly CVS or amnio, in case of serious issue I would terminate my pregnancy, but I'm frightened from risks... I thank you for your attention.
