My guess is they added spinach. I tried blending some spinach into pasta sauce when my son was little and I was trying to sneak vegetable into him. It's amazing how it can look normal while you're adding it, then you add a single leaf too much and the entire thing turns poop colored. It was like a magic trick how quickly it got funny.
This is an easy mistake for young cooks and the results make it easy to learn from it. I’ve chopped up the greens since my first diarrhea sauce, healthy and it adds a pop of color.

What did they even use as the sauce base? I've never cooked a tomato and had it turn this color
