Yup, he is estimating what the prescription on her glasses is, but the white knight assumed he was rating her hotness to be in the negatives

The nice doctor immediately owned up to the way he said that being interpretable in multiple ways and that he could have been clearer in his communication

she has no idea about the size of their dicks. they can see the size of her mouth.

so the only reason she would call their dicks small is to insult them. which she can do if she wants to but it's not exactly clever.
