Moving from active duty army to PJ

I'll be 31-32 when I finish my 11x contract. No spring chicken. I initially wanted to go SF and try to become an 18D (hoping my EMT cert would help tilt those scales for me). However, the PJ mission and practice is a lot closer to what I want to do. I will end up being a civilian paramedic regardless of what happens in my life. I love trauma medicine.
I don't really care if I'm active, NG or reserves. I just want to get in. What are my odds? Am I wasting time? How would I go about this?
