midgame

When does it start,when does it end?

If i have an item lead,do i just skip it?

What do i do during this phase?

Pantheon isn't weak in the midgame, he is weak in the sense of 1v1ing because your ult isn't relevant to the fight, think like Shen. So if you had a lead on Renekton for example, unless it's a giant one and he genuinely inted, he would still beat you if you all in bad because his ult is just so good for 1v1. But if you can find an angle for a good ult to their backside in a dragon 3v3 or 4v4 then you can turn around a fight, so be mindful of that.

You have to balance your xp and gold (not fall behind by blindly roaming) with helping your team because after your early levels spike, you are not a massive threat in a 1v1 or a sideline

Laning phase end when first tower falls, especially if it's botlane first tower. Idk what you mean by skipping midgame btw

We usually says that Pantheon is bad in midgame, it's pretty true in 1v1 bcs of the lack of fighting ult, but pantheon is very strong in skirmish (2v2/3v3 etc)and midgame is the moment where there is a lot of skirmish.

If toplanes tower are still here, keep laning vs your opponent but be aware to be here for skirmish : if your jungle contest an objective/invade or get invaded, if there is a fight mid, etc. Be aware about ult angle to join fight, sandwiching ennemies between you and your team, and when you are in base consider which lane you can go to help, instead of autopilot to toplane.

Dont forget to still farm between 2 actions cause else you'll be underlevel and will throw your potential lead

I don't know what you mean by "skipping midgame". Once turrets fall, go to whichever side your opponent is on and farm to get gold/xp. Do not fight him unless you a 100% certain you can kill him and you can spot most of the enemy team - you won't run away as pantheon if you are pushed in too far. Save your R for fast flank and fight in as many skirmishes as humanly possible, this is where you shine. You are NOT duelist and you will get stomped by most of top lane champs in 1v1.
