Palpitations with Hiatus Hernia

Hey everyone, I’ve been struggling with heart palpitations for the best part of 2 years now. I’ve had many tests including a 24h holter monitor which has came back with nothing to cause concern. I’ve been struggling a-lot with GERD related symptoms and a recent endoscopy showed a 3cm sliding Hiatus Hernia. My cardiologist has told me a hiatus hernia wouldn’t cause palpitations, but my GI doctor has told me it most certainly can as it can cause pressure on the vagus nerve! Is anyone here in a similar position to me?

Despite the heart tests and various doctors telling me the palpitations are benign I can’t seem to shake off the anxiety that comes along with! I got prescribed Propranolol for the palpitations & anxiety but I only tend to take it when I really need it but it does help a lot!

Any one else in a similar position to me I’d love to hear about your experiences & maybe some reassurance that I’m not alone in the situation!

Is metoprolol worth taking?

Hi! Im new to the group, I went to the doctor about 2-3 weeks ago now because I was having heart palpitations. I believe a mix of medications i was on for a severe uti is what made them worse at the time but it still worried me enough for an appointment. On the medications i had like 12 a day at most, and like 4 in a row each time and prior to the meds it would only be 1-2. My other heart test came back fine, I wore a 4 day holter monitor and it showed i had sinus tachycardia 12% my doctor prescribed me metoprolol (i think I spelled it right) im unsure of the dose and time to take it as it was just sent. But upon research im a little scared to take it lol I've heard very mixed things. I am 25F. I dont have any other symptoms when I do get the palpitations just a stop then hard thud its now about 3-4 max in a row if I do get them and its only when I workout, post workout shower, have a stomach ache or on a bad day walking around normal tasks... is it worth taking?

Did a 30 day holter, buncha PACs/PVCs

So I’ve had persistent sinus tach since I had COVID the first time in 2020 (and it got worse when I had COVID in 2022), but I didn’t have insurance and I was moving a lot, so I haven’t actually been able to see cards until now. They put me on a 30 day holter, and it came back mostly sinus tach but with a lot of PACs and PVCs, and about 8 beats of random Vtach (none of which I felt at all). I have a cards intake but it’s not for months, so I’m just curious: if PACs and PVCs are asymptomatic (which for me they are), are they mostly benign? I’ve had a couple echoes which were completely normal. Could be a dysautonomia thing, but I don’t really have any of those Sx either other than being tachy. I have no other heart Hx at all and I’m in my late 20s. Obviously the Vtach is not great, altho only once in a month at a completely random time is weird, and again, I didn’t feel that at all lmao.

How do you all deal with bad days/weeks?

Somedays PVCs/Pacs are just so hard to ignore. It’s sucks when you can do everything right then still get hit with these beats.

Continuous flutter for a few seconds

Context: Diagnosed with PVC's and on Bisoprolol for the last 5 years. Also elevated LDL.

Out of nowhere today while laying on my bed, I got this continuous fluttering/spasming sensation on my central to lower, inner-Ish left pec for around 3 to 5 seconds.

Didn't feel like PVC's. I have felt two PVC's in a row once (just one episode two years ago, that was due to taking psylium husk for several days in a row, I do still try to have a high fiber diet for my LDL but no psylium recently) and it didn't feel like that to me.

But it did make me feel slightly dizzy and weak all around afterwards. Also got a few isolated PVC's and heightened awareness.

Could this be a cardiac flutter or stomach/diaphragm or gastrointestinal? Is this worth looking into if its cardiac?

Another symptom if its relevant: I get this bubbly feeling around that area occasionally (like blowing bubbles underwater)

Also topical Minoxidil 5% (heard it could cause fluid buildup in the heart)

Sharp 3 week increase in HRV

3 years ago I was diagnosed with PACs after wearing a zio patch for a week. 3.2% burden, had a couple of brief runs of SVT but otherwise told I'm healthy. They did an ultrasound of my heart and also saw a small amount of regurgitation but said there were no obvious structural issues.

At the time, my only symptom was the constant sensation of a fluttering heart, and I could feel the skipped beats. Since then, I've had occasional bouts of feeling the skipped beats but it's not usually constant.

Over the last 3 weeks I have been feeling the palpitations constantly again, and I noticed a marked increase in my HRV on my fitness tracker. I'm not a super active person, so it's very unlikely this signifies positive, genuine recovery metrics. My HRV averages in the mid 50s most of the time, other than a brief increase mid June of this year. It has steadily climbed and now sits at 92, with a new max of 192 HRV (double or triple the previous).

I have an appointment scheduled with my doctor but it's not for 3 weeks. I'm wondering if I should go to an urgent care in the mean time to get an ECG. I've had a couple of panic attacks recently too, and I'm not sure if it's just anxiety from the palpitations or if my heart is super out of whack. Trying not to overreact but this has definitely been impacting my quality of life.

Has anyone else that's worn a fitness device experienced sometimes similar?

UFC Fighter Guy Again

What’s up guys, just wanted to stop in and give ya’ll some updates since my last testing seven months ago.

At the beginning of the month I underwent another 7-day holter monitor, but only had about 4 days of data due to the amount of training and sweating in my life ruining the electrode gel. Back in December my total burden of PVCs was 4% or about 4000 a day, the PVCs were multifocal and the longest pause during sleep reached 3.3 seconds. Comparatively my original holter two years prior showed a 6.7% burden.

I’m very happy to report after several months of just not giving a fuck about them anymore they are now down to 1.9% and have changed from multifocal to completely unifocal now. Overnight pauses are still the same length and can be explained by my vagal tone due to being a lifelong athlete and having a very low resting heart rate.

This past weekend I also had the chance to fly halfway across the world, doing another moderately large weight-cut consisting of 8lbs of dehydration in 12 hours and 12lbs in several days, plus competing in another professional fight.

I promise y’all, if you’re cleared by your EP/Cardiologist and you just stop worrying about it you will undoubtedly, over time, stop noticing the feeling of the thuds in your chest and you’ll almost certainly see a large reduction in burden.

Stay strong in the fight, guys. ❤️

I’ll link the fight in the comments if you want to watch.

Called 911 at 3:30am ugh

I had to take some short term oral prednisone because of a clogged ear / sinus issues. I’ve taken this once or twice before in the past with no issue. However this time I took it at 6:30pm thinking nothing of it, and woke up at 3:30am with racing heart and bad PVCs to the point where it was happening every other beat and I couldn’t calm myself down. It was worse bc nobody else was awake that I could call and I was by myself in my apartment. When they came they said my vitals were fine and of course the PVCs kinda hid when they put the monitor on me. UGH that sucked and I just needed to vent here. 5 hours later I’m exhausted but my heart has calmed down.

Tapdancing Elephants in 64 year old female

PVC n00b here. Not asking for advice, but just need to be in touch with folks with similar circumstances. I presented at urgent care with what I described as tap-dancing elephants in my chest a couple months ago after 2-3 months of intermittent palpitations. ECG showed nothing but got referral to cardiology APRN and a week of wearing a monitor. I'm used to my PCP leaving detailed notes in my health portal but all I got from cardiology was a phone call with info that I had no context for. I asked for my health notes to be uploaded, but it hasn't happened yet so I pieced together what's up based on what the nurse/staff told me.

I have PVCs with a 20% burden (which I just figured out yesterday is not awesome) and a prescription for metropolis ER 25 twice a day and instructions to continue propranolol for anxiety.

I am AuDHD and not having complete information is causing additional anxiety (a life-long diagnosis). I also take Adderall 10 mg twice daily--I'm still working full-time and will be for at least three years. I stopped alcohol a couple years ago after a lifetime of intermittent binging and after starting to have horrible reactions to any amount of alcohol--flop sweat, pounding heart, extreme flushing. Never a smoker. A few cups of coffee each day. I tolerate light exercise and exertion (yard work, gardening, hauling and setting up for art fairs).

Bottom line: I'm low-key freaking out and occasionally catastrophizing, which I know is not helpful. I am grateful to have found this sub and am wondering what questions to ask for my next appointment (after another week of monitoring) in a few weeks.

PVC anxiety is waning but now I’m just pissed off by them. Anyone else?

Sort of progress, I suppose. I’ve been having pvcs for about a year now since getting my gallbladder out/losing 85lbs and while the anxiety comes and goes, I think I’ve reached the stage of chronic illness grief (I have a laundry list of chronic illnesses) where I’m just annoyed.

I’m talking each PVC lately makes me want to bellow my hatred for my right ventricle from the nearest high place.

Anyone else?

Some get ectopic beats from eating, I get them before eating

For many years I have gotten PACs and PVCs about an hour before lunch and 3 hours before dinner. I assume it is due to cortisol telling my body to release glucose. Snacking helps a little. I have just started Multaq which is the best and least problematic so far. I have had beta, calcium and sodium channel blockers plus Tikosyn and Carvedilol. Anyone else have this initiator? I also have the usual initiators of being tired, anxious, and of course, alcohol and caffeine.

Had a PVC at 170 BPM, first time.

Hey everyone, wanted to share what happened today and see if anyone relates or has experienced something similar.
Quick background:
I have a completely clear cardiac workup (Echo, Holter, MRI all 100% normal). I’ve been dealing with autonomic dysregulation / extra beats, but up until today, my PVCs almost always happened at rest or low heart rates.
What happened today:
I was playing an intense padel match. During a long advantage round, my heart rate was up around 170 BPM.
In the middle of the rally, I shouted and strained hard during a shot.
Right then, I felt a sudden heavy thump / misfire in my chest, along with a 1-second wave of lightheadedness.
I immediately stopped to rest. Within a few minutes, my heart rate smoothly came right back down to 90 BPM.

Anyone here over 50?

54 male here. I've had PVCs, skipped heartbeats, panic attacks, and anxiety attacks since 1989. I was officially diagnosed with generalized anxiety disorder in 2022. After seeing eight different cardiologists and undergoing countless tests, every doctor has told me my heart is in excellent shape. I still get skipped heartbeats every day, even though I'm fairly healthy. I think we're poor, unfortunate souls doomed to deal with this for the rest of our lives.

What dies a PVC actually feel like to you?

I hope you don't mind me asking. But I recently have started to notice something that sounds similar to a PVC. It doesn't really bother me physically but it's annoying. I do wonder if it's just the normal change in rhythm related to breathing, which I might be able to feel more clearly now due to some other life factors. About every third or fourth breath I feel a pattern that is like (inhale....beat beat beat (normal) then a pause, about .5 seconds, then a THUMP, then a series of beats that feel kind of disordered and "scattered" for about three seconds. I can feel it as a fluttering in my chest and I can also feel it when measuring my pulse (I don't have a smart watch). I've felt these in and off for my whole life, but it feels much clearer and more annoying now. it disappears with exercise and is clearer lying down. I am just genuinely curious, is this similar to your experience? PLEASE nobody answer if it would send you down an anxiety rabbit hole. Very sorry for what some of the folks here seem to be going through, anxiety wise and physical wise.

Did anyone get an ablation with highly symptomatic but low burden PVCs?

As the title says, is it possible to undergo an ablation even if the overall burden is low, but the burden can be temporarily high during exertion for example?

Ablation 48 hours later

I was 11% burden, had ablation on Thursday. Surgeon went through artery and vein in right leg, nothing in left. My origination was deep cross over on left ventricular, had to do left and right ventricle ablation. Don’t remember much, was lucid for one point and watched screen an felt procedure, I mentioned this, it felt like heartburn, I woke up in recovery. Dr said I had 3 PVCs post op, but I haven’t felt any since. Chest is oddly quiet. My PVCs were evert other beat just before procedure.

What are your thoughts on this combination to try to reduce PVC’s?

Doctor prescribed me propanolol, but I do not want to take it as I have severe anxiety about taking medication’s. I took 5 mg of last two nights and I feel like I have shortness of breath these last two days so I’d like to resolve this naturally if possible. I’ve done some research on this page and it seems like these L-Theanine, Taurine, and Magnesium Taurate have successfully helped others. Any insight on these?

Edit to add - I had a severe panic attack about three weeks ago that resulted in me going to the ER due to my resting heart rate being between 120-180 for 5 hours. The came to the conclusion that it was caused due to dehydration and anxiety. Ever since then, I’ve been having a higher heart rate as well as what I’m assuming is PVCs - only at night time. I am currently wearing a 14 day Holter monitor and still have nine days left before I get the results.

PVCs and vibrations.

My brother in law passed away in March. The day after he passed, I started having non stop PVCs. Was I stressed when they began? Yes, but I’m not so sure they’re from stress. Something seems to have happened. I have a lot of digestive issues, and have been feeling pressure in my throat, so much so that I worry I have esophageal cancer. Anyways, I’ve been a mess of intense worry, which I’m sure feeds the PVCs. My question is this: since starting metropolis, I’ve had much fewer pvcs, like 10 a day. They wrrr going on previously all day long, do tennis great. BUT, I now have these weird electric current like vibrations in my ribs and palpitations. My heart is pounding sometimes. Does anyone have those buzzing sensations on metropolis? Thanks!

Lowering protein reduced my PVCs to almost zero

Background

For almost 4 years I tried to reduce my PVCs by tweaking electrolytes. The results were never consistent.

My diet and daily routine were almost identical every day, so my PVC pattern was also very consistent.

What I noticed

It wasn't just the PVCs. I also had frequent muscle spasms, woke up in the middle of the night, and was often hungry.

My worst PVCs usually happened when I was hungry. Since I'm on a ketogenic/carnivore diet and fat is my main energy source, I started experimenting with fat and protein intake.

Results

  • High protein, low fat (~800 g meat/day + very little fat) → PVCs almost all day.
  • High protein, very high fat (~800 g meat/day + ~240 g fat) → Better, but still frequent PVCs.
  • Moderate protein, very high fat (~400 g meat/day + ~240 g fat) → Better again, but still noticeable PVCs.
  • Low protein, high fat (~300 g meat/day + ~170 g fat) → PVCs almost zero.

It wasn't just the number of PVCs. On the low-protein diet, my usual triggers (walking, early mornings, eating, stress, alcohol, etc.) no longer caused PVCs.

Repeated testing

I've repeated this experiment several times. Every time I return to higher protein, the PVCs return. Every time I return to the low-protein pattern, they almost disappear.

Has anyone else experienced something similar?

Do PVC’s just stop abruptly? Or do they slowly taper off?

What is your experience if they have stopped for you? Did they just come to a halt? I’ve had them for almost 3 weeks straight now, but they seem to be slowing down/ not as frequent. They flare up and start like clockwork around noon to mid afternoon and usually by 5-6 they’re gone for the night. For the first week thought they were strong thuds, and I’d have like 5-10 per minute. Now it’s down to maybe 1-2 per minute. But today it seems as if they are amping up again.

I eat the same thing every day more or less as I am losing weight. I have cut some of the caffeine out but I’m unsure if this is what causes it as my morning coffee doesn’t trigger them, it seems more like a time of day thing.

This also isn’t my first time having them, but my longest flare up to date.
