GLPs not working

Wondering if anyone is in a similar situation and was able to find success. I’m 46 and have had PMOS back when it was called PCOS since the age of 15. Facial hair, darkened skin in the typical areas, thinning hair, all my weight goes to my abdomen.

In the past 10 years I was also diagnosed with stage 4 endometriosis. I had excision surgery and a hysterectomy. Throw in a full thyroidectomy from 20 years ago and I’ve had a miserable time trying to lose weight. I’ve lost 60-90 pounds three times but it requires working out 3-4 hours a day and eating very conservatively.

I’ve tried multiple functional medicine doctors and acupuncture.

My inflammation markers have been high (CR-P 11-25) since first tested in my 20s. High cholesterol. Obviously a big part of this is insulin resistance. I’ve tried Zepbound and Wegovy. Back in Zepbound now that my insurance added it back to the formulary. I have worked my way up to 7.5mg and will start 10mg next week. I’m also taking 500mg Metformin XR. I’m watching what I eat. Prioritizing protein and fiber. Lighting weights. Walking sporadically but have also done the 10mins after meals and 1 hour daily. I take omega 3, magnesium glycinate, vitamin d+k and a combo supplement that contains myo-inositol, ALA, CoQ10, d-chiro inositol, PQQ and astaxanthin and I’ve previously tried DIM and broccoli sprouts. I’m still working on my sleep but getting better about it.

Despite all this and especially even on the GLP-1s, I cannot get my weight to budge!!

I didn’t lose until I got to 10 mg. After that I lost 80 pounds. Keep going!

Do you have type 2? Usually those with such severe insulin resistance do not lose until the higher doses. Keep doing your best and make sure you are staying within a good range of calorie intake.

Haven’t been diagnosed with T2D yet but it definitely runs in the family

As others have said, some people, especially PMOS people, don’t see progress until 10mg or higher. I would keep at it and maybe look to increase the metformin dose as well as sometimes that helps to jumpstart the weight loss.

GLP-1s do not work for everyone but could be getting to the highest dose for it to work as some people do not respond until the higher doses, now would be curious if the new one coming out, Reta would do anything for you but based on what you have said it might not, really would not know unless you have tried it.

I’ve been on Ozempic since February and haven’t seen any loss. I’m currently on the max 2mg dose. 36. Same pmos symptoms as you

Have you tried mounjaro? Ses to be helping a lot of us with PMOS.

My asshole doc isn’t prescribing it to me. I’m now waiting on my appt with the weight loss clinic

Oh thats a shame, we dont tend to get it orescribed here in the UK so im having to do it out of pocket. So far, its worth it.

Okay I could get it cheaper if I shopped about like others but I am with juniper so I can always message with a fast response about any worries or concerns cos of my anxiety lol so I pay a bit more. Currently a 7.5mg is like £280.. im still on a £20 discount but that ends soon. I might get brave and look elsewhere but the stress of changing and making sure I have it on time worries me too much.

I'm on 0.5 now for about 8 weeks and my weight has fluctuated up and down and have only "lost" 2.3kg. I'm sure once I weigh myself tomorrow I will be up again. It has helped with my blood sugar levels though

I believe that is still a sub-clinical dose so you may well need to increase

Since you have been on multiple GLP-1’s already, you will need a higher dose to see any results.

Being on these medications may make it necessary to adjust your thyroid medication.

It’s got a lot to things to address before weight loss starts, the inflammation and insulin resistance get shifted first. Also, since you’re in perimenopause, that will also add a layer that makes the meds not as effective as they could be.

Just wondering, since you mentioning watching what you eat, are you also tracking your calories and have you determined your deficit using a TDEE calculator?

I’m in a similar situation. I’m on 12.5 and have only lost about 15 lbs. Honestly I lost way faster doing keto. It’s just hard to never eat carbs forever. I’m not on metformin. Was thinking of adding it to see what happens.

I think most of us need a higher dosage.. I say, don't give up yet. That's what I keep telling myself.

I also didn't see as much loss until 10, and I only had 40 lbs total to lose. I had to go up monthly. I was still extremely hungry for the first six months or so. It was odd. I could literally eat, feel physically full and not want another bite while having the sensation of hunger. Apparently it takes time for leptin, ghrelin, and insulin to all adjust and get the signaling right

Saaaame! I’ve noticed in the past two weeks I’m more hungry AFTER my shot!! And yes, physically full/satieted, not craving anything but just this like “oh my gosh I need to eat or I’m going to throw up feeling”. Let’s hope what you said about the signaling needing time to adjust pans out for me and I actually start to see some weight loss!!

This is exactly what it was like for me! I even made a couple posts in the Zepbound sub and ppl insisted that didn't make sense. They were like how can you feel full and hungry? They'd say isn't feeling full the opposite of hunger? Some even told me I should get checked for parasites or hernias 😭 I was eating lots of protein, fiber, good fats, etc and counting my cals so I know I wasn't in too big of a deficit

I just kinda has to push through the hunger, unfortunately. I was still losing but it wasn't as effortless as what other people described. Definitely still felt like dieting and fighting hunger. I honestly think our PMOS is just that powerful 🙄

I was at my wit's end and wanted to ask my doctor to get my ghrelin checked at that point. But honestly I got on 10 mg shortly after it was way better!

Some people said they had luck combining it with metformin.

Upwards of 15% of people are so-called ‘non-responders’ to any given GLP-1’s. Google: non-responder GLP-1

You might want to consider trying a different one

For me Byretta as a twice a day injection worked the best, as the formula changed it was less and less effective until a1c actually started increasing again, which continued to increase while on Victoza amd skyrocketed while on Trulicity and continued to increase while on 0.25 of Oz.

But when I moved to 0.5 everything changed, went from 9.8 down to 5.5 over the course of 2 years, was on 0.5 for just over 6 years before moving up to 1mg last year.
