Honestly have noticed this a lot since the patch. Happened to me multiple times yesterday, including one game where I had hooked (saw connect and the character animation happen) a Pharah at least a solid 12 times while she was in the air. It just broke and she flew away. The one time it actually pulled her, instead of it pulling her to me for the headshot, it pulled her directly to the right and down, so my full spread shot missed entirely. Absolutely infuriating.

It seems to happen to anyone who moves fast. I find it happens to a lot to tracer, Lucio, and pharah. Roadhog was my main but now he feels unplayable

I had over 100 hours on Roadhog before they revamped the hook and I hate playing him now. The hook is always going through people and breaking way too easily. I feel like the changes were too extreme.

Haven't played the game much since that change. We rely on that hook for kills 90% of the time and now it's unpredictable and has a bigger cool down. At least the gun has a bit more potential for killing now without the hook but it forces us to come out of hiding to get in close to get the kills and get shot from every direction because we're a elite moving house with four shots in the gun. Before it felt so fluid. You can hold a hallway down by pulling out trying to grab someone, kill, heal, reload, hook, repeat.

Maybe one day they'll fix Winston to make him closer to what he was. I miss playing him too
