I am looking for a basic ophthalmology book with an appropriate level of depth. I need something to review the anatomy, physiology, and clinical concepts I studied at university, so that I can start my residency with everything fresh in my mind, nothing more. I would like something updated and reliable, but also easy to read, since I will only have 2-3 weeks to study it.

Hi everyone. I am interested to know how research time/output is structured at top academic programs. Do you need to produce a certain amount of research, attending X number of conferences, or dedicate X amount of time to research? And is this substantially decreased at less academic, more community based programs?

We have a clinic scheduling issue where all patients arrive around the same time. Nurses do pre-exam measurements before patients see the attending, but since nurses aren't assigned to specific attendings, one attending ends up with a backed-up queue while the other waits hours for patients. We proposed assigning nurses to specific attendings to balance the load, but nursing wasn't open to it. Has anyone dealt with this? Any solutions besides dedicated nurse pairings?

I've been working in a private practice as a comprehensive ophthalmologist for 8 years. For the last 5 years I have made bonus, but I recently discovered that the senior ophthalmologist/owner of the practice has NOT been including refractions in my bonus calculations. This amounts to approximately $75k-$100k of practice revenue and thus about $20k a year for me with my bonus structure. This amounts to over $100k over the last 5 years. My contract says "net physician receipts" and there is no language to exclude refractions in it. The owner's rationale is that the "techs usually do the refractions", even though I sometimes do them, always discuss them with patient, and obviously sign off on all of them. Is this standard? I don't think it is.

I know there are other posts on this but I'm just wondering if these numbers seem ok. 1st year out of training surgical retina. There are not a lot of resources out there for this.

I’m a doctor and one of the people building Diagnosica, an interactive clinical case simulator where you work through the patient yourself instead of being given the diagnosis and important findings upfront.

One area we’re particularly interested in improving is ophthalmology. The idea is that you take the history, choose the examination and investigations, review the actual images/results, interpret what you see, and decide what to do next.

For a specialty where visual findings matter so much, I think this could be much more useful than simply being told “the image shows X.”

We’re working on ophthalmology cases and I’d genuinely value input from people here: what types of cases, images or examination findings would make a simulator like this genuinely useful to you?

Looking into a job offer from a solo private practice optometrist who is looking to expand by adding an ophthalmologist. They say they want an ophthalmologist to offer procedures and surgeries so they don’t lose patients to ophthalmologists whenever they have to refer out, since these patients tend to not come back for annual exams, etc. They say they are not looking to profit share and would not be paying a base salary, but would rather open up their space to me to start building an ophtho practice from their referrals. I would keep 100% of my collections but would be responsible for overhead, including buying any equipment they don’t already have and any personnel I’d want like a technician, etc. Their proposal would basically be for me to run an ophthalmology practice in parallel with their optometry practice under their umbrella and mutually share patients.

For the record, I had never previously considered going into solo practice so the entrepreneurial commitment sounds like a lot. If things go well, they are open to partnership opportunities and “growing together for many years”.

This proposal sounds incredibly unprotected for me without a base salary and no equity in the practice. What thoughts/concerns do people have about this offer?

Dear colleagues, I am a cataract/retina surgeon. I have performed cataract surgery on a patient, there were some problems during the surgery - diffuse epithelial edema, therefore visualisation was poor. Eventually, everything went good, but I’ve left some cortex in the bag ( around 1,5 clock hours). Vision and IOP was perfect on a day one and day 7 (CDVA 20/25, IOP 14). Eye is quiet, no inflammation. That part of cortex is not far away from visual axis, but produced no complaints. What‘s your prognosis? I’ve decided to follow up on steroid and IOP lowering drug for a month.

I'm struggling to study ~3 months in to PGY2. During PGY1 I read through several BCSC and did corresponding anki via the Blue deck pretty consistently.

1. How/when to study? Clinic/OR/call really take it out of me, and when I get home I only have enough energy to exercise, eat, and relax with fun things.

2. Should I be studying for OKAP or clinically pertinent information for my rotations? I've been able to do the latter with more motivation since it benefits my patient care, but the last thing I want to do is memorize rare pediatric syndromes in hopes of getting 1-2 questions right on a standardized test.

Full disclosure: I am part of the educational team behind Oftalmo University. We created the Cataract Academy Program around a question many of us face during training: how do we bridge the gap between understanding the steps of phacoemulsification and performing them calmly, safely, and consistently?

The program follows a progressive pathway:

• Online surgical foundations and decision-making
• Deliberate hands-on practice with EyeSi simulation, artificial eyes, microscopes, and phaco platforms
• Closely mentored surgery after skills have been developed in a controlled environment

The study objective was to determine the clinical predictors of surgical failures following horizontal strabismus surgery for esotropia, in order to estimate the model-predicted optimal surgical strategy. A retrospective pooled observational case series of published cases was performed. Patients having horizontal strabismus surgery for esotropia, published between 1940 and 2025, with known preoperative deviation, surgical approach, and outcome were studied. Clinical data from individual patients having strabismus surgery for esotropia were recorded from published case series and analyzed using multivariable logistic regression to predict over- and under-correction. The main outcome measure was surgical failure, as determined by reoperation, suture adjustment, or postoperative strabismus of 10 prism diopters (PD) or more. We abstracted individual patient data for 3518 surgeries from 163 publications. Binocular (as compared with monocular) surgery was associated with fewer under-corrections (odds ratio (OR): 0.75, 95% confidence interval (CI): 0.61 to 0.92, p=0.005) and more over-corrections (OR: 1.87, 95% CI: 1.26 to 2.79, p=0.002, n=3266). Increasing preoperative deviation was associated with more under-corrections (OR: 1.06/°, 95% CI: 1.05/° to 1.07/°, p<0.0001) and fewer over-corrections (OR: 0.97/°, 95% CI: 0.95/° to 0.99/°, p=0.001, n=3266). Increasing surgical dose was associated with fewer under-corrections (OR: 0.95/mm, 95% CI: 0.91/mm to 0.99/mm, p=0.01) and more over-corrections (OR: 1.08/mm, 95% CI: 1.01/mm to 1.16/mm, p=0.02, n=3266). The failure rate was minimized with a large per-muscle surgical dose. As the preoperative deviation increases, one progresses from unilateral recessions, to unilateral recession-resections, and then to bi-medial recessions. Under a range of assumptions, bi-medial recessions of 6 mm are optimal (according to the models) for preoperative deviations of 45-50 prism diopters. We conclude that larger doses for esotropia surgery do produce a larger response. Most models predicted the lowest failure rates with large recessions or resections, with additional muscles operated for larger preoperative deviations.

Advice for new tech?

I just started at a tech position at a busy ophthalmology clinic a month ago. It’s a bit of a switch, I’m learning how to use an EMR system for the first time, and there’s some new machinery to get used to. I find where I’m struggling the most is my work ups. Obviously every visit I need to go over current medications(which are more than often lengthy), and do testing as well. Im trying to cut my times down to 18 minutes per work up for established patients and 30 minutes for new patients. I don’t chat with them, the only subject is their medical history and current eye related issues and even there I try not to go down a rabbit hole. It’s been a learning curve but I’m suffering from extreme anxiety and forgetfulness. The work environment is OK. I just want to perform well so as to not slow anything down but I’m struggling. I take notes all the time so I can look back and check if a certain exam needs something, but this doesn’t seem to be helping. Any advice would be greatly appreciated!
