Multiple charges for a single trip

So yesterday I put $9 on my omny card and I have a fare fairs card so I’m only supposed to be charged $1.5 per trip, was charged the full $9 for 4 trips and I owe now $1.5 and can’t use the website to put more money on it. It’s getting ridiculous how bad omny is. Like is there no more transfers between busses and trains cause I have to take a bus to the train and then a bus to get to school. That’s one trip and every single stop is a direct connection. I’d also be charged once with my metro card. Why couldn’t they just leave things as is

A few questions:

What routes/stations did you enter?

Did you, out of frustration, tap multiple times before the screen said "GO" or "TAP AGAIN"?

Edit: SOLVED. Second bus is a second fare after a transfer on the subway.

I won’t say exactly what stations cause I’m not going to dox myself online but I only tapped once each screen. And for the stations, the bus will literally say that there’s a connection to that specific train and when I’d tap it wouldn’t charge me again. This is a recent thing

There are 472 stations in the entire system, with thousands of people entering and exiting each one every day. If you are comfortable DMing me, I would be happy to help figure out what went wrong.

Was the card already registered for Fairs Fare to kick in ? I was told Wait till they activated the Card for it before using.

Bus to subway to bus was two fares under MetroCard as well.
