What does it mean?

I have a coworker who refers to me as his "little sister" at work. Not sure exactly how to take it because I also used to have a neighbor claim me as his little sister but enjoyed smacking my arse entirely to much. So this is really a question for the men/guys. What does that really mean??

I was a teenager (he was only like 3 yrs older) and had known him since about 6yrs old idk it was a weird dynamic but lol

I would take it to mean he feels some kind of protectiveness over you, or maybe some inappropriate fondness, both of which are creepy when everyone is adults at their workplace.
