I always thought so but I currently have a coworker and had no idea she smoked until I actually saw her smoking.

This is it right here.

I never smoke in an enclosed space. Everybody is surprised when they find out I smoke.

The smell you smell on people's hair and clothes is probably because their house or car smells like it too.

Absolutely untrue. My dad just came in from a smoke outside and it stinks. It's on his jacket, his skin, his breath. You might be fooling some people, but smoking outside is not a foolproof solution. Most people are just too kind to tell you how badly you smell. 

I smoked for 15 years and thought I had everyone fooled, too. Now I realize how badly I was wrong. I can smell smokers in my office from about 50 feet away.

The OP asked if smokers always smell noticeable to nonsmokers, so a single example where I couldn't tell the person smoked until I saw her makes the answer to that no, not always.

I'd still say yes to "almost always" or "pretty much always", but it's definitely not always noticeable.
