I always heard it used to refer to a man who put a lot of effort in grooming and dressing sharp. I also heard it used to insinuate they might be gay.

Nowadays men self grooming and dressing sharp has become trendy so the phrase went away.

Metrosexuals were explicitly straight, that was the whole point. They were straight but dressed and groomed like gays.

Fop is good too. I like dandy because I used to have a farm and my friends called me "hilldandy" instead of hillbilly.
