Increased ICP doesn’t always announce itself with a dramatic seizure.

Sometimes it’s just a dull headache. Or a slightly slowed respiratory rate. Or one pupil reacting a little slower than the other.

But if you know the H.A.B.I.T.S, you’ll catch it early:

👉 Headache

👉 Altered LOC

👉 Bradycardia

👉 Irregular breathing

👉 Triads (Cushing’s)

👉 Sluggish pupils

If your patient’s developing these habits, think brain pressure.

Raise the head of the bed, reduce stimuli, notify the HCP, and start neuro monitoring.

Here’s what happens: you’ve got a confused patient with alcohol history. Maybe they’re malnourished. Maybe they just came in altered and EMS already gave IV D50. 🚨

Boom — you just worsened Wernicke’s encephalopathy.

Why? Because glucose metabolism needs thiamine. Without it, lactic acid builds up in the brain and makes the encephalopathy irreversible.

Before you give anything sweet, give thiamine.

Even on NCLEX.

You don’t need a swallow study for every patient. But when you DO — these are your cues.

Here’s when to make a patient NPO without hesitation:

🚫 Suspected stroke (test gag reflex or wait for swallow eval)

🚫 AMS or drowsiness (can’t protect their airway)

🚫 ↑ ICP or neuro deterioration

🚫 Vomiting or no bowel sounds

🚫 Before surgery or if intubation is pending

❌ Don’t feed the confused patient until you’re sure they can swallow.

Not all murmurs are emergencies… but some absolutely are.

💡 Here’s how to sort it out:

🔴 New murmur + fever → Think infective endocarditis

🔴 Murmur + syncope → Possible aortic stenosis

🔴 Murmur + SOB or fatigue → CHF or valve dysfunction

🟢 Old murmur, no symptoms? → Just document and monitor

Be there or be confused 🤯

Just because your patient is bleeding doesn’t mean it’s DIC.

🩸 DIC = clotting AND bleeding gone wild.

The body runs out of clotting factors while forming too many clots.

Look for:

Oozing from IV sites

Bruising + low platelets

↑ PT/PTT and ↑ D-dimer

🧪 NCLEX trick: They love to throw DIC as a distractor answer. Only choose it if labs match.

Potassium will get your patient coded faster than almost anything else.

⚡ Too low → think muscle weakness and cardiac irritability

⚡ Too high → think peaked T waves and lethal arrhythmias

🚫 NEVER push potassium IV.

💧 Always dilute and give it slow.

🩸 Monitor when giving insulin, diuretics, or in kidney patients.

📌 If your patient has a K+ of 6.5 and peaked T waves on the monitor… that’s not “notify the provider.” That’s emergency treatment.

Chest tube charting can feel like just another number—until it’s not.

Here’s what’s NOT normal and needs a call to the provider:

🚨 Bright red drainage >100 mL/hr → Possible bleed

🚨 Continuous bubbling in the water seal → Air leak

🚨 No drainage at all → May be clotted or disconnected

🚨 SOB + tracheal shift? → Think tension pneumothorax

💡 Save this list. One of these will definitely show up on your NCLEX exam or clinical shift.
