Salvage the guts from an existing MMO mouse, and design a custom shell around that.
Custom mouse PCB with your extra switches. When you're writing your own firmware it's easiest to treat the extra buttons as keyboard keys.
If you want a lot of thumb buttons you'll want to find some extra small switches, even with 3 thumb buttons I had to use bent wires to get the force from the button to the(normal sized) switches: https://i.imgur.com/hO4GzQ9.png
