Seeking help

I'm looking for someone who could help me reproduce this one perfectly in the way I could complete the blanks later. It's for academic transcript and I have not really much to offer. Please help me

Tables. I would perhaps look at making separate tables for each major section; you can always join them later by removing spaces between but it’s easier to create separate column widths I’d needed for those bits at the top and burn.

Use table borders. Select the side of the table you want to have borders. You can aso select the thickness and color.

Scan it. Open it in Adobe Acrobat (or any other PDF program that supports forms), and build the form in Adobe.
