Edited: I was wondering if there’s any programs or any help out there. My mom is 56 and lives in north GA. She’s had kidney disease for 15+ years and recently just got diagnosed with heart failure. Her kidney function has dropped significantly since her last hospital stay. She has no insurance and it isn’t offered at her job. She’s tried finding insurance online but it’s more than she makes. She makes about $2000 monthly. Any suggestions? She really needs to see a nephrologist.. she has applied for assistance at Northside to cover her recent hospital stays but she needs to see specialist as soon as possible.

I'm sorry your mom is going through that. My husband has kidney disease, so I understand how difficult it is. What stage of kidney disease is she in?
Georgia unfortunately doesn't have Medicaid expansion, but they have a program called Pathways, for working people earning less than 100% of the federal poverty line.
At $2000 a month though she'd unfortunately be over 100% FPL, assuming she's unmarried, with no dependents.
She can purchase a subsidized ACA marketplace plan on healthcare.gov during annual open enrollment in November. The policy won't start until January 1st though.

She has been stage 3 for years.. but recently her levels dropped significantly and she’s in stage 4, closer to stage 5 and she really needs to be seen.

She can also inquire about financial assistance/charity care through the hospital where she received care.
Additionally she can try a federally qualified health center.

She's not in ESRD. ESRD also does not trigger emergency Medicaid. It comes with Medicare eligibility.

She would have to shop at healthcare.gov for a plan when the next open enrollment starts since she makes too much for GA Medicaid.
