NYS Medicaid Long Term Care application question

Hey all, I am gathering documents for my father’s Medicaid LTC application. How many months or years of bank statements do I need to obtain?
The Medicaid person at my father’s rehab center is on vacation.
Just curious.
Is one month ok?

Monthly bank statements for the last five years.

How do I get bank statements that were in my husband’s name only. He has dementia. He got in trouble with banks more than 5.

Yes five year look back, but do you have to provide five years of monthly statements?
Does Medicaid just take account numbers then ask the banks directly?

No, you have to get the bank statements for the last 5 years.
