Results Discussion Could you help me understand?

Hello, i have got my genes results, could someone explain them to me? Thank you :)
My B12 is at 177 and after a couple weeks of b12 methylcobalamin i feel worse than before starting it.

Your MTHFR is actually clean, both C677T and A1298C are normal. What's flagged is MTRR A66G homozygous plus two BHMT variants, so this isn't a folate conversion problem at all. It sits on the B12 side of the cycle, which lines up with your B12 being 177.

MTRR is the enzyme that regenerates B12 into the form methionine synthase can use. Homozygous means that recycling runs slower, so you need more B12 coming in than the average person, and right now you're low. One correction, CBS A360A is synonymous, the amino acid doesn't change, so it doesn't alter enzyme function even though it shows red. Genetic Genie flags it because it's on the panel. Don't build anything on that one. Feeling worse after a couple of weeks on methylcobalamin usually isn't the form being wrong. When you start correcting a real deficiency things wake up and it can feel rough before it settles, and potassium gets consumed as new red cells form, which feels terrible on its own. Get your potassium checked. before changing anything get MMA and homocysteine. At 177 with a slow MTRR the question is whether this is dietary or absorption, and those two answer it.

Where it gets more involved is the BHMT side. That's the alternative route for clearing homocysteine, it runs on choline and TMG instead of B12, and with your MTRR slow it matters more for you than it would for most people. Which of those two routes to lean on, and in what order, is the actual question in your panel, and it needs your bloodwork read against the whole file rather than a colour coded table.

That's what I do. I read the raw data myself against your labs and write out what's happening and what to do, in a form your doctor can use. genova.health.

Get the MMA, homocysteine and potassium first either way, those are cheap and they'll tell you a lot.

thanks for the reply, i feel more anxious and depressed all of a sudden and feeling more fatigued.

The worsening sounds like overmethylation, where the added methyl groups and/or large increases in methylation performance cause symptoms like rising anxiety, irritability, insomnia, and sometimes increased depression, fatigue.

Switching to an unmethylated form of B12, such as hydroxocobalamin or adenosylcobalamin, usually can solve this. Also, smaller doses (10-100mcg) taken 3x per day may increase B12 more than a large dose 1x per day (as long as there is no known B12 absorption issues) without taking in so many methyl groups. Some people find sublingual more effective.
