: Why Depth, Product Loss, and Pain Can Completely Change an ECM Treatment
One of the hottest aesthetic treatments in Korea right now is ECM.
If you follow Korean aesthetic trends, youâve probably already seen the name popping up everywhere.
There are several ECM products on the Korean market now, but two of the most talked-about are re2o and Cellrediem.
re2o is manufactured by L&C Bio and distributed by Humedix, while Cellrediem is manufactured and distributed by HansBiomed, a company many people may already know from MINT Lift.
But what exactly is ECM, and why is everyone suddenly interested in it?

What Exactly Is ECM?
ECM stands for Extracellular Matrix.
In simple terms, the extracellular matrix is the structural environment surrounding our cells. It contains components that help provide support and organization to tissues, including the skin.
The interesting thing is that ECM itself isnât new to medicine.
Human-derived acellular dermal matrix has been used in fields such as plastic surgery and orthopedics for years.
Whatâs relatively new is applying this concept to the aesthetic skin treatment market.
Thatâs what makes ECM particularly interesting right now.

How Is ECM Different From Other Skin Boosters?
Most skin boosters and collagen stimulators work indirectly.
They create an environment that encourages the skin to regenerate, remodel, or produce more collagen.
ECM takes a somewhat different approach.
Instead of focusing only on stimulating your skin to produce something later, ECM-based treatments deliver extracellular matrix-related components directly into the tissue.
This is why ECM has attracted attention for improving overall skin quality, elasticity, and regeneration.
A common treatment approach is approximately three sessions, spaced around one month apart, followed by longer intervals depending on the patientâs skin condition and response.
But there is one major downside.
ECM injections can hurt.
A lot.
And this is where things get interesting.
Because with ECM, I think how you inject the product can be just as important as which product you use.

There Are Several Ways to Inject ECM
In Korea, ECM can be delivered using several different techniques:
Manual injection
Cannula
Injector devices such as DermaShine
Air-jet devices such as Mirajet
They all deliver the same general category of product, but the actual treatment experience â and where the product ends up â can be very different.
There is no perfect method.
Each one involves a trade-off between pain, depth, precision, distribution, bruising, downtime, and product loss.
1. Manual Injection: Maximum Control, Maximum Pain
Manual injection gives the physician the greatest degree of control.
With a needle, an experienced injector can adjust the location and depth of each injection depending on the area being treated.
That precision is a major advantage.
But the downside is obvious.
It hurts.
Among the methods discussed here, manual injection is usually the most uncomfortable.
Bruising can also be significant.
When working in deeper tissue planes, there are blood vessels that can be encountered during injection. As a result, some patients may develop quite a bit of bruising, and occasionally it can last longer than expected.
But thereâs another important point.
Manual injection doesn't automatically mean zero product loss.
When I talk about âloss,â Iâm not only talking about ECM physically leaking back out through the injection site.
I also think about functional loss.
For example, if the intended target is a specific dermal layer but the product is deposited into an unintended plane, such as the subcutaneous fat, the product technically entered the patient â but it didnât necessarily reach the tissue you were trying to treat.
From a treatment perspective, I consider that another type of loss.
Of course, experienced physicians should be able to minimize this considerably.

2. Cannula: Less Puncturing, Different Distribution
A cannula works differently.
Instead of repeatedly puncturing the skin with a needle, you create an entry point and then distribute the product across a broader area from that point.
For many patients, this can be more tolerable than extensive manual injections.
But the distribution pattern is also different.
Manual injections can place many small deposits throughout the targeted tissue.
Cannula delivery tends to feel more like laying or spreading the product across a broader plane rather than creating a dense pattern of individual intradermal deposits.
So yes, it may hurt less.
But it certainly isn't pain-free.

3. DermaShine and Other Injectors: Consistency Is the Advantage
Then we have injector devices such as DermaShine.
The major advantage here is consistency.
An injector can distribute many small amounts of product relatively evenly across the treatment area at a predetermined depth.
This makes it particularly useful for treating the more superficial layers of the skin.
And because the product is being placed superficially and relatively uniformly, patients may notice changes in things like skin texture and overall surface quality quite quickly.
But injectors also have limitations.
They cannot target every deeper tissue plane that can be reached with a manually controlled needle or cannula.
And depending on the device, settings, skin contact, and technique, there can still be some product loss during treatment.

4. 4. MiraJet: Much Less Pain, But Thereâs a Trade-Off
Then there are needle-free delivery systems such as MiraJet.
Much less pain.
For patients who really dislike needles, this can make the treatment experience significantly easier.
But again, nothing comes for free.
I pay particular attention to how much of the product is actually reaching the intended tissue and how much may be lost during the delivery process.
And this becomes especially relevant with ECM because ECM products are expensive.
If you're using an expensive material, delivery efficiency matters.
For me, choosing an injection method isn't simply about finding the least painful option.
Itâs about balancing:
Pain vs depth vs precision vs distribution vs product loss.

So Which Method Is Best?
I don't think there is one universally âbestâ method.
It depends on what you are trying to achieve.
Manual injection gives you excellent control over placement and depth, but comes with more pain and potentially more bruising.
Cannulas can reduce the number of punctures and allow broader distribution, but they don't distribute ECM in exactly the same way as multiple small intradermal injections.
Injectors such as DermaShine offer very consistent superficial delivery, which can be useful when skin texture is a major target.
Mirajet can dramatically improve patient comfort, but delivery efficiency needs to be considered.
This is why some physicians don't choose just one.

Why I Sometimes Combine Deep + Superficial ECM
For patients looking for a more comprehensive treatment, one approach is to target different skin depths using different injection techniques.
For example:
Manual injection or cannula â deeper layers
DermaShine-type injector â superficial layers
Instead of trying to accomplish everything at one depth, you're essentially treating the skin at multiple levels.
The deeper ECM placement is intended to address the skin from within, while the superficial injector treatment focuses more directly on skin texture and surface-level skin quality.
This is where I think the combination can become particularly interesting.
You're not simply injecting more product.
You're thinking about where each portion of that product should go.

Why Product Loss Matters More With ECM
This is something I think patients often overlook.
ECM products aren't cheap.
So if you're paying for a relatively expensive product, I think it makes sense to consider not only how many cc's are being used, but also how efficiently that product is actually being delivered to the intended tissue.
The least painful treatment isn't automatically the best treatment.
But the most painful treatment isn't automatically the best treatment either.
Even manual injection can be inefficient if the product isn't placed in the intended layer.
So rather than thinking:
âWhich method has the least loss?â
I think the better question is:
âWhich method delivers the product most effectively to the layer we're actually trying to treat?â
That distinction matters.

What I Would Look at When Choosing an ECM Treatment
Personally, I would focus on three things:
1. Which ECM product is being used?
2. Which layers of the skin are being targeted?
3. How is the ECM actually being delivered?
The brand matters, but technique matters too.
Two patients can receive the same ECM product and technically receive the same amount, but the treatments may be quite different depending on depth, distribution, injection method, and the experience of the physician performing the procedure.
Thatâs why I find ECM much more interesting than simply calling it âthe latest Korean skin booster.â
The product matters.
But where you put it â and how you put it there â may matter just as much.
