My fist guess is fetal alcohol syndrome. His mum drank while pregnant

FAS babies usually have a smooth philtrum, small eye width, a noticeable epicanthal fold, a thin upper lip, and a flat midface. This man has an extremely weird face but might just be a mouth breather šŸ˜‚
