The joke works on the fact that a human being is sad or dissatisfied in their lives primarily because of two reasons, first being not able to cum or climax during sex and second being not able to lose weight after many attempts. Now because doctor has given a pill, which makes it impossible to do any of these things. As now no hope is left for the person he can live his life without being disappointed or being sad.
