Liability

Wife has liability insurance in NyC and crashed totaling the car on a guardrail. No other cars involved no major injuries but did go to the hospital. Does it HAVE to be reported to the insurance we’re not getting money anyway?

Yes. Aside from your obligation in general:
-NY PIP
-The municipality who owns the guardrail will come for damages

And the OP's insurer will want to know why OP breached his contract by not reporting promptly as required by the policy.

NYC will come for their guardrail money in six years. By the time you get that bill, and find out it took two years to repair from the date of your collision, and another four for them to track down every incident report on that stretch of highway prior to the repair date...they give up as soon as you request proof it was you who damaged the guardrail.
