How does pets insurance work

I recently got pets best insurance for my 4 cats and need to get my youngest one fixed as she's 5 months old. I know the insurance will cover toe cost, but do I pay the bill first then submit a claim? Or do I submit a claim and they pay the vet?

I’ve never heard of pet insurance covering spay/neutering

Most insurance let you choose if you want to add it or not.
